天津市河西区480例社区8月龄婴儿智力发育筛查及影响因素分析

摘　　要：目的 对天津市河西区480例社区8月龄婴儿进行智力筛查,同时对影响8月龄婴儿智力发育的影响因素进行分析,以期为智力发育异常的预防和早发现、早干预提供更多依据.方法 选取2019年10月-2022年10月在天津市河西区东海街社区卫生服务中心进行智力发育筛查的天津市河西区社区480例8月龄婴儿作为研究对象;采用丹佛智能发育筛查量表对480例8月龄婴儿进行智能发育筛查,同时采用该院自制《8月龄婴儿智能发育影响因素调查表》对影响因素进行调查.结果 480例8月龄婴儿中正常婴儿占比97.50%(468例),可疑婴儿占比0.21%(1例),智力发育异常婴儿占比2.29%(11例),可疑和异常均判定为智力发育筛查阳性,此次智力发育筛查阳性率为2.50%(12例).单因素分析结果显示:母亲生产时年龄、父亲年龄、母亲受教育年限、出生时胎龄、出生体重、是否合并宫内窘迫均为婴幼儿智力发育的影响因素.结论 天津市河西区8月龄婴儿智力发育总体情况良好,8月龄婴儿智力发育受母体条件、生产因素、婴儿自身因素等多种因素影响,临床应针对影响8月龄婴儿智力发育的高危因素进行产前、生产过程及产后的多方干预,以降低婴儿智力发育异常的发生率,同时也有利于对高危儿的跟踪筛查,实现早发现、早治疗.Objective To screen and influence factors of intelligence development in 480 cases of 8-month-old infants Hexi District Tianjin City in order to provide more basis for the prevention,early detection and early intervention of mental development abnormality.Methods From October 2019 to October 2022,480 cases of 8-month-old infants were selected from the community of Hexi District,Tianjin,Denver intelligence development screening scale was used to screen the intelligence development of 8-month-old infants,and the influence factors were investigated by the self-made"8-month-old infant intelligence development questionnaire".Results Among the 480 cases of 8-month-old infants,97.50%(468 cases)were normal infants,0.21%(1 case)were suspicious infants,and 2.29%(11 cases)were abnormal infants.Both suspicious infants and abnormal infants were diagnosed as intelligence development screening positive,the positive rate of intelligence development screening was 2.50%(12 cases).The results of univariate analysis showed that maternal age at delivery,paternal age,maternal education,gestational age at birth,birth weight and intrauterine distress were the influencing factors of infant intelligence development.Conclusion The overall intelligence development of 8-month-old infants in Hexi District,Tianjin is good.The intelligence development of 8-month-old infants in Tianjin is influenced by many factors,such as maternal condition,production factors and infant's own factors,in order to reduce the incidence of abnormal intellectual development of infants,we should intervene the high risk factors of intellectual development of 8-month-old infants before and after delivery,at the same time,it is also beneficial to follow-up screening of high-risk infants,early detection and early treatment.
