四步逐层顶椎旋转复位法治疗青少年特发性脊柱侧弯的临床观察

摘　　要：目的 探讨四步逐层顶椎旋转复位法治疗青少年特发性脊柱侧弯的临床疗效.方法 选择2021年1月-2023年9月河池市人民医院符合青少年特发性脊柱侧弯临床诊断标准的门诊患者90例,随机分为观察组及对照组A、对照组B,每组各30例,观察组采用四步逐层顶椎旋转复位手法治疗,对照组A采用四步逐层顶椎旋转复位手法配合Boston矫形支具外固定治疗,对照组B仅采用Boston矫形支具外固定治疗.在治疗4个月后,对比分析三组患者治疗前后Cobb角的变化、VAS评分及临床疗效的改善情况.结果 三组患者治疗4个月后,Cobb角均较治疗前明显减小(P<0.05),观察组和对照组A患者治疗后Cobb角明显小于对照组B,差异有统计学意义(t=4.132,P=0.000);观察组和对照组A总有效率明显高于对照组B,差异有统计学意义(χ^(2)=3.714,P=0.036;χ^(2)=3.265,P=0.032);观察组、对照组A治疗4个月后VAS评分明显低于对照组B(P<0.05).结论 四步逐层顶椎旋转复位法治疗青少年特发性脊柱侧弯疗效显著,为青少年特发性脊柱侧弯的中医规范治疗提供了依据.Objective To investigate the clinical effect of four-step top vertebral rotation reduction in the treatment of adolescent idiopathic scoliosis.Methods A total of 90 outpatients meeting the clinical diagnostic criteria for adolescent idiopathic scoliosis in the People's Hospital of Hechi City from January 2021 to September 2023 were selected and randomly divided into observation group and control group A and control group B,with 30 cases in each group.The observation group was treated with four-step parietaleal rotational reduction manipulation.Control group A was treated with four-step rotating reduction of the top vertebrae and external fixation with Boston orthopedic brace,control group B was treated with external fixation with Boston orthopedic brace only.After 4 months of treatment in each group,the changes of Cobb Angle,VAS score and improvement of clinical symptoms before and after treatment in 3 groups were compared and analyzed.Result After 4 months of treatment,Cobb Angle decreased significantly in all groups compared with before treatment(P<0.05),the Cobb Angle of observation group and control group A was significantly lower than that of control group B after treatment,and the difference was statistically significant(t=4.132,P=0.000).The total effective rate of observation group and control group A was significantly higher than that of control group B,and the difference was statistically significant(χ^(2)=3.714,P=0.036;χ^(2)=3.265,P-0.032);after 4 months of treatment,the VAS of observation group and control group A was significantly lower than that of control group B(P<0.05).Conclusion The four-step top-vertebra rotation reduction method is effective in the treatment of adolescent idiopathic scoliosis,which provides a basis for the standard treatment of adolescent idiopathic scoliosis in traditional Chinese medicine.
