血常规在不同类型疟疾诊疗中的临床应用

摘  要:目的探索不同疟原虫感染对血细胞的影响,探索区分不同疟原虫感染以及预测疗效的血液检测指标。方法选取2013年10月至2020年1月期间就诊于苏州市第五人民医院确诊疟原虫感染的患者,系统分析血常规各项指标与疟原虫类型以及疗效间的关系。结果疟原虫感染会影响血常规多项指标,恶性疟和间日疟患者外周血淋巴细胞比率及数量均低于健康对照组,且经治疗后恶性疟患者淋巴细胞数能明显上升;疟原虫感染患者红细胞数量、血红蛋白量以及体积均低于健康对照组,并且经治疗后恶性疟患者红细胞数和细胞体积能明显上升;恶性疟和间日疟患者外周血外周血血小板数量均明显低于健康对照组,同时发现恶性疟患者血小板低于间日疟患者,进一步分析还发现恶性疟患者治疗后血小板数量明显升高且差异具有统计学意义,而间日疟患者治疗前后血小板数量上升较不明显。结论不同类型的疟原虫感染对外周血细胞的影响不同,淋巴细胞计数、红细胞以及血小板可以为疟疾的分类诊断和治疗效果评估提供依据。Objective To explore the effect of different plasmodium infection on blood cells,and to investigate the blood detection indexes to distinguish different plasmodium infection and predict the therapeutic effect.Methods Patients diagnosed with plasmodium infection were selected from the Fifth People’s Hospital of Suzhou from October,2013 to January,2020,and the relationship between blood routine indexes and plasmodium type and therapeutic effect was systematically analyzed.Results Plasmodium infection could affect multiple blood routine indicators.The proportion and number of lymphocytes of malaria patients were lower than that of the healthy control group,and the number of lymphocytes in falciparum malaria patients increased significantly after treatment.The number of erythrocytes,hemoglobin and volume of patients with malaria were lower than that of the healthy control group,while the number of erythrocyte and volume of patients with falciparum malaria increased significantly after treatment.The number of platelets in the peripheral blood of the patients with falciparum malaria and tertian malaria was significantly lower than that of the healthy control group,and the number of platelets was found to be lower in patients with falciparum malaria than in the patients with tertian malaria.Further analysis also showed that the number of platelets in patients with falciparum malaria was significantly increased after treatment,while the number of platelets in patients with tertian malaria was not significantly increased before and after treatment.Conclusion Different types of plasmodium infection have different effects on peripheral blood cells.Lymphocyte count,red blood cells and platelets can provide a reference basis for the classification and diagnosis of malaria and the evaluation of therapeutic effect.
