摘 要:目的研究早产黄疸患儿血清γ-谷氨酰转肽酶(γ-GT)、转铁蛋白(TRF)、白蛋白(ALB)水平及与蓝光治疗效果的关系。方法选取2020年1月至2022年1月医院收治的早产儿黄疸患儿103例作为研究对象为实验组,选取同期出生的100例正常健康新生儿作为对照组。早产黄疸患儿均采用蓝光治疗,比较不同黄疸类型(病理性、生理性)、不同病情程度(轻度、中度、重度)早产黄疸患儿血清γ-GT、TRF、ALB、总胆红素水平差异;记录患儿治疗前后血清γ-GT、TRF、ALB、总胆红素水平变化;按治疗效果分为有效组和无效组,比较不同蓝光效果早产患儿血清γ-GT、TRF、ALB、总胆红素水平;分析早产黄疸患儿血清γ-GT、TRF、ALB水平与总胆红素水平的相关性。结果治疗前,实验组TRF、ALB水平明显低于对照组,血清γ-GT水平、总胆红素水平明显高于对照组(P<0.05);与生理性黄疸组相比,病理性黄疸组血清γ-GT、总胆红素水平更高,TRF、ALB水平更低(P<0.05);不同病情程度早产儿血清γ-GT、TRF、ALB、总胆红素水平比较差异有统计学意义(P<0.05),随着病情加重,患儿血清γ-GT、总胆红素水平上升(P<0.05),TRF和ALB水平下降(P<0.05);无效组患儿治疗前及治疗后3、5d总胆红素均比治疗有效组患儿高,TRF、ALB水平均比治疗有效组患儿低(P<0.05);且有效组患儿治疗后3、5d血清γ-GT水平、总胆红素均比治疗前降低,TRF、ALB水平均比治疗前升高(P<0.05);早产黄疸患儿血清γ-GT水平与总胆红素水平呈正相关(P<0.05),TRF、ALB水平与总胆红素水平呈负相关(P<0.05)。结论早产黄疸患儿血清γ-GT、总胆红素水平高于正常新生儿,而TRF、ALB水平低于正常新生儿,且上述指标变化与黄疸病情严重程度密切相关,可指导早产黄疸患儿蓝光治疗效果评估及病情监测。Objective To study the relationship between serum levels ofγ-glutamyltranspeptidase(γ-GT),transferrin(TRF)and albumin(ALB)and effect of blue light therapy in premature children with jaundice.Methods 103 premature children with jaundice admitted to the hospital from January,2020 to January.2022 were selected as the experimental group,while 100 normal healthy neonates born during the same period were included in the control group.All premature children with jaundice were treated with blue light.Serum levels ofγ-GT,TRF,ALB and total bilirubin were compared among premature children with jaundice of different types of jaundice(pathological and physiological)and different disease severities(mild,moderate and severe).Changes in serumγ-GT,TRF,ALB and total bilirubin levels were recorded before and after treatment.According to the treatment effect,these children were divided into the effective group and ineffective group,and serum levels ofγ-GT,TRF,ALB and total bilirubin were compared among premature children with different blue light effect.The correlation between serumγ-GT,TRF and ALB levels and total bilirubin level in premature children with jaundice was analyzed.Results Before treatment,levels of TRF and ALB in the experimental group were significantly lower than those in the control group,while levels of serumγ-GT and total bilirubin were significantly higher than those in the control group(P<0.05).Compared with the physiological jaundice group,serum levels ofγ-GT and total bilirubin were higher,while levels of TRF and ALB were lower in the pathological jaundice group(P<0.05).There were significant differences in levels of serumγ-GT,TRF,ALB and total bilirubin among premature infants with different disease severities(P<0.05).With the aggravation of disease,levels of serumγ-GT and total bilirubin were increased(P<0.05),while levels of TRF and ALB were decreased(P<0.05).The total bilirubin in the ineffective group before treatment and at 3 and 5 days after treatment was higher than that in the effective gro
