增强T2⁃FLAIR序列在颅内转移瘤诊断价值

摘　　要：目的:对比增强T2⁃FLAIR序列及增强T1WI序列,探讨增强T2⁃FLAIR序列在脑转移瘤中的应用价值。方法:收集2021年1月—2022年12月本院收治的经病理确诊为原发性肿瘤、同时确诊或怀疑有脑转移的患者58例,患者在进行传统的MRI平扫及增强检查的基础上,加做增强T2⁃FLAIR序列。观察2种增强序列图像上转移瘤的数量、大小、部位及转移瘤的强化显示程度,并进行统计学分析。结果:本组58例患者,增强T2⁃FLAIR序列显示的部分脑实质结节状转移及软脑膜转移的强化程度,显著高于增强T1WI序列,差异具有统计学意义(P<0.05),脑实质结节转移的直径增强T2⁃FLAIR序列较增强T1WI序列大,差异具有统计学意义(P<0.05)。结论:增强T2⁃FLAIR序列对于软脑膜的转移及皮质区的小结节状转移灶有很高的检出率,对常规的MRI增强起到很好的补充作用,两者联合使用可以显著提高脑转移瘤的诊断准确性。Purpose:To compare enhanced T2-FLAIR sequence with enhanced T1WI sequence,and to explore the application value of enhanced T2-FLAIR sequence in intracranial metastatic tumor.Methods:From January 2021 to December 2022,58 patients with pathologically confirmed primary tumor and brain metastasis were collected.On the basis of traditional MRI plain scan and enhanced examination,patients were given enhanced T2-FLAIR sequence.The number,size,location and enhanced display degree of metastatic tumors on two kinds of enhanced sequence images were observed and statistically analyzed.Results:In this group of 58 patients,the enhancement degree of nodular metastasis and meningeal metastasis of brain parenchyma showed by enhanced T2-FLAIR sequence was significantly higher than those by enhanced T1WI sequence,with a statistically significant difference(P<0.05).The diameter of nodular metastasis of brain parenchyma was larger by enhanced T2-FLAIR sequence than that by enhanced T1WI sequence,with a statistically significant difference(P<0.05).Conclusion:Enhanced T2-FLAIR sequence has a high detection rate for meningeal metastasis and small nodular metastasis in cortical area,which is a good supplement to conventional MRI enhancement.The combined use of T2-FLAIR sequence can significantly improve the diagnostic accuracy of brain metastases.
