摘 要:目的:探讨薄层小视野非压脂T2WI联合弥散加权成像(DWI)技术对宫颈癌术前MRI分期诊断的准确性。方法:126例经活检证实的宫颈癌患者纳入研究,术前MRI检查采用薄层小视野非压脂T2WI及DWI序列,以术后病理为对照,计算术前MRI分期的特异度、灵敏度、阴性预测值(NPV)、阳性预测值(PPV)和准确度。采用Kappa检验分析宫颈癌术前MRI分期与术后病理的一致性。结果:MRI分期ⅠB1、ⅠB2、ⅠB3、ⅡA1、ⅡA2和ⅢC1分别为23例、36例、18例、25例、17例和7例,病理分期ⅠB1、ⅠB2、ⅠB3、ⅡA1、ⅡA2和ⅢC1分别为21例、38例、15例、24例、18例和10例。MRI对于宫颈癌ⅠB1、ⅠB2、ⅠB3、ⅡA1、ⅡA2和ⅢC1分期的准确度分别为98.41%、100%、97.62%、94.44%、89.68%和94.44%,总准确度为95.77%,与病理结果有较好的一致性(Kappa值=0.842,P<0.001)。结论:薄层小视野非压脂T2WI联合DWI对宫颈癌术前分期的诊断效能较高,具有临床推广价值。Purpose:To explore the accuracy of thin layer small field-of-view(FOV)non fat-suppressed T2WI combined with diffusion-weighted imaging(DWI)in preoperative MRI staging diagnosis of cervical cancer.Methods:A total of 126 patients with biopsy-confirmed cervical cancer were included in the study.Preoperative MRI was performed using thin layer small FOV non fat-suppressed T2WI and DWI sequences.With postoperative pathology as the reference,the specificity,sensitivity,negative predictive value(NPV),positive predictive value(PPV),and accuracy of preoperative MRI staging were calculated.The consistency between preoperative MRI staging and postoperative pathology staging was analyzed using the Kappa test.Results:The MRI staging forⅠB1,ⅠB2,ⅠB3,ⅡA1,ⅡA2 andⅢC1 were 23,36,18,25,17 and 7 cases,respectively,while the pathological staging forⅠB1,ⅠB2,ⅠB3,ⅡA1,ⅡA2 andⅢC1 were 21,38,15,24,18 and 10 cases,respectively.The accuracy of MRI staging forⅠB1,ⅠB2,ⅠB3,ⅡA1,ⅡA2 andⅢC1 of cervical cancer were 98.41%,100%,97.62%,94.44%,89.68%and 94.44%,respectively,with an overall accuracy of 95.77%,showing good consistency with the pathological results(Kappa value=0.842,P<0.001).Conclusion:Thin layer small FOV non fat-suppressed T2WI combined with DWI has high diagnostic efficacy for preoperative staging of cervical cancer and has clinical value for promotion.
