摘 要:目的探究实时超声弹性成像联合灰阶超声在甲状腺占位性病变诊断中的应用价值。方法前瞻性选取2021年12月至2022年12月在我院接受治疗的106例甲状腺占位性病变患者为作为研究对象,均实施实时超声弹性成像、灰阶超声诊断,分析2种方法诊断结果。结果106例患者经手术病理诊断,40例为恶性病变,66例为良性病变。联合诊断组诊断准确率高于单独灰阶超声诊断组,差异有统计学意义(χ^(2)=5.62,P<0.05)。联合诊断组诊断灵敏度、特异度明显高于单独灰阶超声诊断组,差异有统计学意义(χ^(2)=5.68,5.90,P<0.05)。恶性病变患者与良性病变患者血流分级情况整体比较,差异有统计学意义(χ^(2)=5.62,P<0.05)。恶性病变患者的血流阻力指数明显高于良性病变患者,差异有统计学意义(χ^(2)=10.98,P<0.05)。结论甲状腺占位性病变患者可使用实时超声弹性成像联合灰阶超声诊断,有效提高诊断准确率,鉴别良性病变、恶性病变。Objective To explore the application value of real-time ultrasound elastography combined with gray-scale ultrasound in the diagnosis of thyroid space-occupying lesions.Methods One hundred and six patients with thyroid space-occupying lesions treated in our hospital from December 2021 to December 2022 were prospectively selected as the research objects,and real-time ultrasound elastography and gray scale ultrasound diagnosis were performed.Analyze the diagnostic results.Results One hundred and six patients were diagnosed by surgical pathology,40 malignant and 66 benign lesions.The diagnostic accuracy of the combined diagnosis group was higher than that of the gray-scale ultrasound diagnosis group alone,which was statistically significant(χ^(2)=5.62,P<0.05).The combined diagnosis group had significantly high diagnostic sensitivity and specificity in the single gray scale ultrasound diagnosis group,which was statistically significant(χ^(2)=5.68,5.90,P<0.05).Overall comparison of flow distribution between patients with malignant and benign lesions(χ^(2)=5.62,P<0.05).The flow resistance index in patients with malignant lesions,and the difference was statistically significant(χ^(2)=10.98,P<0.05).Conclusion Patients with thyroid space-occupying lesions can be diagnosed by real-time ultrasound elastography combined with gray scale ultrasound to effectively improve the diagnostic accuracy and distinguish benign and malignant lesions.
