摘　　要：目的 探讨自动痔疮套扎术(ruiyun procedure for hemorrhoid,RPH)治疗环状混合痔的临床疗效及安全性。方法 方便选取2020年2月—2022年2月苏州大学附属太仓医院收治的82例环状混合痔患者为研究对象,利用随机数表法分为观察组(RPH联合外痔切除术)和对照组(痔上黏膜环切术联合外痔切除术),各41例。对比治疗效果。结果 两组的治疗有效率对比,差异无统计学意义(P>0.05);观察组手术时间明显少于对照组[(25.05±9.19)min vs (39.12±10.37)min],差异有统计学意义(t=6.502,P<0.001);观察组术中出血量明显少于对照组,差异有统计学意义(P<0.05);与对照组比较,观察组术后24、72 h、1周疼痛评分、切口恢复时间、术后并发症总发生率均更低,差异有统计学意义(P均<0.05)。结论 环状混合痔接受RPH治疗的临床疗效较佳,患者的症状可得到明显改善,且并发症少。Objective To investigate the clinical efficacy and safety of ruiyun procedure for hemorrhoid(RPH) in the treatment of annular mixed hemorrhoid.Methods A total of 82 patients with annular mixed hemorrhoids admitted to Taicang Hospital Affiliated to Soochow University from February 2020 to February 2022 were conveniently selected as the research objects.According to the random number table method,they were divided into observation group(RPH combined with external hemorrhoidectomy) and control group(prolapse and hemorrhoidectomy combined with external hemorrhoidectomy),with 41 cases in each group.The therapeutic effects were compared.Results There was no statistically significant difference in the effective rate between the two groups(P>0.05).The operation time of the observation group was significantly less than that of the control group [(25.05±9.19) min vs(39.12±10.37) min],the difference was statistically significant(t=6.502,P<0.001).The intraoperative blood loss of the observation group was significantly less than that of the control group,and the difference was statistically significant(P<0.05).Compared with the control group,the pain scores at 24,72 h and 1 week after operation,the recovery time of incision and the total incidence of postoperative complications in the observation group were lower,and the differences were statistically significant(all P<0.05).Conclusion RPH treatment of annular mixed hemorrhoids has better clinical efficacy,the symptoms of patients can be significantly improved,and there are few complications.
