苦参汤加味坐浴熏洗联合胰岛素对糖尿病患者患炎性外痔的临床疗效探讨

摘　　要：目的探讨苦参汤加味坐浴熏洗联合胰岛素治疗糖尿病患者患炎性外痔的临床疗效。方法选取2022年2—12月连江县医院收治的60例糖尿病患者作为研究对象,按随机数表法分为治疗组30例和对照组30例。治疗组采用苦参汤加味坐浴熏洗联合胰岛素与外痔切除术治疗,对照组采用苦参汤加味坐浴熏洗联合二甲双胍缓释片与外痔切除术治疗。观察两组手术后恢复情况、疼痛程度、血糖水平、术后并发症。结果两组术后创面愈合、首次排便及住院时间比较,差异无统计学意义(P均>0.05)。两组术后48、72 h的疼痛数字评估量表评分比较,差异无统计学意义(P均>0.05)。治疗前两组空腹血糖及餐后2 h血糖水平比较,差异无统计学意义(P均>0.05);治疗后治疗组5、10 d空腹血糖及餐后2 h血糖水平均低于对照组,差异有统计学意义(P均<0.05)。治疗组不良反应发生率低于对照组,差异有统计学意义(P<0.05)。结论苦参汤加味坐浴熏洗联合胰岛素与外痔切除术治疗糖尿病患者患炎性外痔具有较好的临床疗效,可作为一种新颖的治疗方法。Objective To investigate the clinical effect of flavoured Kushen decoction plus sit-bath fumigation com-bined with insulin in the treatment of diabetic patients with inflammatory external hemorrhoids.Methods 60 patients with diabetes treated in Lianjiang County Hospital from February to December 2022 were selected as the study objects and divided into treatment group(30 cases)and control group(30 cases)according to random number table method.The treatment group was treated with flavoured Kushen decoction combined with insulin and external hemorrhoidec-tomy,while the control group was treated with flavoured Kushen decoction combined with metformin sustained release tablets and external hemorrhoidectomy.The postoperative recovery,pain,blood glucose level and postoperative com-plications were observed in the two groups.Results There was no statistically significant difference in wound healing,first defecation and hospital stay between the two groups(all P>0.05).There was no statistically significant difference in Numerical Rating Scale scores between the two groups at 48 and 72 h after operation(both P>0.05).There was no statistically significant difference in fasting blood glucose level before treatment and 2 h after meal between the two groups(both P>0.05).After treatment,the levels of fasting blood glucose and 2 h after meals at 5 and 10 days in the treatment group were lower than those in the control group,and the differences were statistically significant(all P<0.05).The incidence of adverse reactions in treatment group was lower than that in control group,and the difference was statistically significant(P<0.05).Conclusion Flavoured Kushen decoction combined with insulin and external hemorrhoids has good clinical effect in the treatment of diabetic patients with inflammatory external hemorrhoids,and can be used as a novel treatment method.
