摘　　要：目的探讨尿蛋白、尿微量蛋白联合检测在糖尿病患者早期肾损伤中的诊断价值。方法选择2021年3月—2022年6月福建医科大学附属龙岩市第一医院收治的63例糖尿病合并早期肾损伤患者纳入观察组,同期选择收治的50例单纯糖尿病患者纳入单纯糖尿病组,另同期选择63例体检中心体检健康者纳入对照组,入组患者均进行尿蛋白、尿微量蛋白检测,比较各组患者尿蛋白、尿微量蛋白计数等指标水平,同时比较尿蛋白、尿微量蛋白单独检测及联合检测的诊断效能。结果不同分组患者尿蛋白、尿微量白蛋白水平比较,差异有统计学意义(F=1161.017、302.506,P均<0.05),且观察组患者尿蛋白、尿微量白蛋白水平高于单纯糖尿病组与对照组患者,差异有统计学意义(P均<0.05)。同时尿蛋白、尿微量蛋白联合检测特异度及灵敏度均高于尿蛋白、尿微量蛋白单一检测值,差异有统计学意义(P均<0.05)。结论临床糖尿病合并早期肾损伤患者诊治中采用尿蛋白及尿微量蛋白联合检测在早期诊断上具有较高价值,进而为患者治疗提供依据。Objective To explore the diagnostic value of urinary protein and urinary microprotein combined detection in early renal injury of diabetic patients.Methods 63 patients with diabetes mellitus combined with early renal injury admitted to Longyan First Hospital Affiliated to Fujian Medical University from March 2021 to June 2022 were in-cluded in the observation group.During the same period,50 patients with simple diabetes mellitus were selected to be included in the simple diabetes group,and 63 healthy patients in the physical examination center were selected to be included in the control group.Urine protein and urine microprotein were detected in all enrolled patients.The levels of urinary protein,urinary microprotein count and other indicators were compared among all groups,and the diagnos-tic efficacy of single detection and combined detection of urinary protein and urinary microprotein was compared.Results There were statistically significant differences in urinary protein and urinary microalbumin levels among dif-ferent groups(F=1161.017,302.506,all P<0.05).The levels of urinary protein and urinary microalbumin in observa-tion group were higher than those in simple diabetes group and control group,and the differences were statistically sig-nificant(both P<0.05).At the same time,the specificity and sensitivity of urine protein and urine microprotein com-bined detection were higher than those of urine protein or urine microprotein single detection,and the differences were statistically significant(all P<0.05).Conclusion The combined detection of urinary protein and urinary micropro-tein in the diagnosis and treatment of patients with diabetes mellitus complicated with early renal injury has a high value in early diagnosis,and thus provides a basis for the treatment of patients.
