摘  要:目的 应用整体纵向应变及峰值应变离散度参数评价不同透析时间的终末期肾病患者的左心室收缩功能的变化。方法 选择2019年1月至2022年12月于河南省人民医院规律行血液透析的患者共82例,按照透析时间分为3组,A组38例(透析时间<1 a),B组24例(1 a≤透析时间≤2 a),C组20例(2 a<透析时间<5 a),选取同时期30例健康人群为正常组,比较正常组与异常组以及异常组间的一般临床资料、二维超声基本参数、整体纵向应变(GLS)、峰值应变离散度(PSD)。结果 A、B、C 3组的GLS及PSD与正常组相比差异有统计学意义,且B组GLS的绝对值高于A组,C组的PSD值高于B组(P<0.05)。结论 规律的血液透析在一定程度上可以有效改善左心室收缩功能,GLS联合PSD参数评价左心室功能的变化更敏感。Objective To evaluate the changes in left ventricular systolic function in end-stage renal disease patients of different dialysis ages using global longitudinal strain and peak strain dispersion parameters.Methods A total of 82 patients who underwent regular hemodialysis at Henan Provincial People’s Hospital from January 2019 to December 2022 were selected.They were divided into three groups according to different dialysis time.There were 38 cases in group A(dialysis time<1 year),24 cases in group B(1 year≤dialysis time≤2 years)and 20 cases in group C(2 years<dialysis time<5 years).Thirty healthy people in the same period were selected as normal group.The general clinical data,basic parameters of two-dimensional ultrasound,global longitudinal strain(GLS)and peak strain dispersion(PSD)were compared between the normal group,abnormal group,and abnormal group.Results The GLS and PSD of groups A,B,and C showed statistically significant differences compared to the normal control group,and the absolute value of GLS in group B was higher than that in group A,the PSD value in group C was higher than that in group B(P<0.05).Conclusion Regular hemodialysis can effectively improve left ventricular systolic function to a certain extent,and the GLS combined with PSD parameter is more sensitive in evaluating changes in left ventricular function.
