摘 要:目的:通过嗅觉功能评估及鼻部相关检查,分析不同原因所致嗅觉障碍患者的治疗效果及评价指标。方法:选择2018—2022年就诊于山东第一医科大学第二附属医院的289例与鼻部疾病相关的嗅觉障碍患者为研究对象。根据造成嗅觉障碍的不同,分为鼻-鼻窦炎嗅觉障碍组、外伤性嗅觉障碍组、上感后嗅觉障碍组。对此各组患者治疗前后的嗅觉功能、鼻功能、鼻窦CT Lund-Mackay评分和鼻内镜检查Lund-Kenned评分。并采用Pearson相关性分析3组患者嗅觉功能检测与鼻功能、鼻窦CT Lund-Mackay评分和鼻内镜检查Lund-Kenned评分之间的相关性。结果:鼻-鼻窦炎嗅觉障碍患者121例(41.8%),外伤性嗅觉障碍患者63例(21.8%),上感后嗅觉障碍患者105例(36.4%)。3组患者治疗前后嗅棒测试评分比较,差异均有统计学意义(P均<0.05)。鼻-鼻窦炎嗅觉障碍患者鼻功能检查、鼻窦CT Lund-Mackay、鼻内镜检查Lund-Kenned与嗅棒测试评分呈正相关(P<0.05);外伤性嗅觉障碍患者鼻功能检查评分与嗅棒测试评分呈正相关性(r=0.292,P=0.036);上感后嗅觉障碍患者鼻功能检查评分与嗅棒测试评分呈正相关(r=0.190,P=0.043);鼻-鼻窦炎嗅觉障碍和上感后嗅觉障碍患者经治疗后,改善率更高(81%,92%),外伤后嗅觉障碍患者经治疗后,效果不够理想,且失嗅率较高。结论:鼻部疾病相关的嗅觉障碍患者通过积极的对因治疗,嗅觉功能可获得改善。嗅觉功能检测可以与鼻功能、鼻窦CT及鼻内镜检查一同作为鼻部相关嗅觉障碍患者嗅觉功能评估和嗅觉功能恢复的评价指标。Objective To analyze the treatment efficacy and evaluation indicators for patients with olfactory dysfunction caused by different reasons through olfactory function assessment and nasal-related examinations.Methods A total of 289 patients with olfactory dysfunction related to nasal diseases who were treated at the Second Affiliated Hospital of Shandong First Medical University from 2018 to 2022 were selected as the research subjects.According to the different causes of olfactory dysfunction,they were divided into the nasal-sinusitis olfactory dysfunction group,traumatic olfactory dysfunction group,and post-upper respiratory tract infection olfactory dysfunction group.The olfactory function,nasal function,CT Lund-Mackay score of the nasal sinuses,and nasal endoscopy Lund-Kennedy score of patients in each group before and after treatment were compared.Pearson correlation analysis was used to analyze the correlation between olfactory function tests and nasal function,CT Lund-Mackay score,and nasal endoscopy Lund-Kennedy score among the three groups of patients.Results There were 121 cases(41.8%)of nasal-sinusitis olfactory dysfunction patients,63 cases(21.8%)of traumatic olfactory dysfunction patients,and 105 cases(36.4%)of post-upper respiratory tract infection olfactory dysfunction patients.The differences in olfactory stick test scores before and after treatment in the three groups were statistically significant(P<0.05).In nasal-sinusitis olfactory dysfunction patients,nasal function examination,CT Lund-Mackay score,and nasal endoscopy Lund-Kennedy score were positively correlated with olfactory stick test scores(P<0.05);in traumatic olfactory dysfunction patients,nasal function examination score was positively correlated with olfactory stick test scores(r=0.292,P=0.036);in post-upper respiratory tract infection olfactory dysfunction patients,nasal function examination score was positively correlated with olfactory stick test scores(r=0.190,P=0.043).The improvement rate of olfactory-sinusitis and postupper respir
