身痛逐瘀汤治疗糖尿病周围神经病变临床疗效的Meta分析

摘　　要：目的评价身痛逐瘀汤治疗糖尿病周围神经病变(diabetic peripheral neuropathy,DPN)的临床有效性。方法计算机检索2000年~2023年1月间VIP、CNKI、CBM、PubMed、Web of science等数据库关于身痛逐瘀汤治疗糖尿病周围神经病变的随机对照试验,利用RevMan5.4软件进行分析。结果纳入10篇文献,共761例患者。Meta分析显示试验组临床总有效率优于对照组[OR=3.60,95%CI(2.43,5.31),P<0.00001];试验组腓总运动神经[MD=2.79,95%CI(2.01,3.57),P<0.00001]和感觉神经传导速度[MD=1.66,95%CI(0.88,2.45),P<0.0001]、正中运动神经[MD=3.34,95%CI(2.33,4.34),P<0.00001]和感觉神经传导速度[MD=1.32,95%CI(0.38,2.26),P=0.006]快于对照组,试验组视觉模拟评分法(visual analogue scales,VAS)评分[MD=-1.09,95%CI(-1.35,-0.83),P<0.0001]、总胆固醇水平[MD=-1.06,95%CI(-1.73,-0.39),P=0.002]低于对照组。结论身痛逐瘀汤可提高治疗糖尿病周围神经病变的临床疗效,提高腓总神经、正中神经传导速度,降低VAS评分和总胆固醇水平,无严重不良事件发生。Objective To evaluate the clinical efficacy of Shentong Zhuyu Decoction in the treatment of diabetic peripheral neuropathy(DPN).Methods By searching VIP,CNKI,CBM,PubMed,Web of science and other databases,we collected and collated the randomized controlled trials of Shentong Zhuyu Decoction in the treatment of diabetic peripheral neuropathy from 2000 to January 2023.RevMan 5.4 software was used to analyze dates.Results 10 publications with 761 patients were included.The Meta-analysis showed that the total clinical effective rate in the experimental group was better than that in the control group[OR=3.60,95%CI(2.43,5.31),P<0.00001],and the common peroneal motor nerve[MD=2.79,95%CI(2.01,3.57),P<0.00001]and common peroneal sensory nerve conduction velocity[MD=1.66,95%CI(0.88,2.45),P<0.0001],median motor nerve[MD=3.34,95%CI(2.33,4.34),P<0.00001]and median sensory nerve conduction velocity[MD=1.32,95%CI(0.38,2.26),P=0.006]were faster than those in the control group.The visual analogue scales(VAS)scores[MD=-1.09,95%CI(-1.35,-0.83),P<0.0001]and total cholesterol level[MD=-1.06,95%CI(-1.73,-0.39),P=0.002]in the experimental group were lower than those in the control group.Conclusion Shentong Zhuyu Decoction can improve the clinical efficacy in the treatment of diabetic peripheral neuropathy,increase the conduction velocity of common peroneal nerve and median nerve,reduce VAS score and total cholesterol level,and don’t casue serious adverse events.
