褚玉霞教授运用褚氏安胎汤治疗早期先兆流产的临床经验举隅

摘　　要：早期先兆流产是临床常见的妇产科疾病之一,其中,阴道出血、腰酸、腹痛、下坠是本病的四大症状。随着三孩政策的开放及女性生存压力的不断增长,本病发病率呈逐年上升趋势。早期先兆流产不但影响孕妇精神心理健康,妨碍妊娠的继续和胎儿的正常发育,甚则威胁母子生命安全。历来医家认为先兆流产多与肾虚、气血亏虚、血热与血瘀有关,主要病机为冲任受损,胎元不固,对该病的治疗也多以补肾安胎为大法。褚玉霞教授是国家级名老中医,从事妇科临床及教学工作五十余载,精研经典,博采众长,充分发挥中医学的优势。褚教授从“虚”“热”认识该病,虚指脾肾亏虚,热为阴虚致热。临床主要针对“虚、热”病理机制,以“补益脾肾,养阴清热,止血安胎”等思路为基本治则,临床处方用药灵活严谨,创制褚氏安胎汤。以补益脾肾为本,养阴清虚热,寒热并投,攻补兼施,气血同调,治病与安胎并举,既注重补益早期先兆流产患者肾虚之本,又顾护后天脾胃,兼清阴虚内热,效如桴鼓,且安全性高,值得临床传承及推广。Early threatened abortion is one of the common clinical obstetric and gynecological diseases,among which vaginal bleeding,backache,abdominal pain and tenesmus are the four major symptoms of this disease.With the opening of the three-child policy and the increasing survival pressure of female,the incidence of this disease has been increasing year by year.EarlyE threatened abortion not only affects the mental health of pregnant women,hinders the continuation of pregnancy and normal development of the fetus,but even threatens the life safety of mothers and children.Historically,doctors believe that threatened abortion is mostly related to kidney deficiency,Qi(气)and blood deficiency,blood heat and blood stasis,and that the main pathogenesis of the disease is the damage of Chong meridian(冲脉)and Ren meridian(任脉),resulting in weak vitality of fetal growth and development,and the treatment of the disease is mostly based on tonifying the kidney for preventing miscarriage.Professor CHU Yuxia is a national famous veteran TCM practitioner,who has been engaged in gynecology clinical and teaching work for more than 50 years.Professor CHU has studied classics conscientiously,learned from others,and given full play to the advantages of TCM to understand the disease from deficiency and heat.Professor CHU believes that deficiency refers to spleen and kidney deficiency,and that heat is caused by Yin(阴)deficiency.Based on the pathological mechanism of deficiency and heat in clinical practice,the basic therapeutic principles are mainly nourishing spleen and kidneys,nourishing Yin and clearing heat,and hemostasis and miscarriage prevention.The clinical prescription flexible and rigorous,and the CHU’s Antai decoction(褚氏安胎汤)is created.The treatment method is based on tonifying spleen and kidneys,nourishing Yin and clearing deficiency-heat,tonification and purgation in combination to regulate Qi and blood at the same time,which not only pays attention to the root of kidney deficiency in early threatened abortion p
