从中医脾胃思想论治脑病的思想探析

摘　　要：中医脑病发病机制复杂,病情多变,风火易为患,危急重症较多,个体差异大且变化迅速,严重者危及患者生命,故不能守一方、一证论治,在诊治时须辨证求因、贵乎精详、以法选方,通权达变。根据脾胃-脑相关理论,结合脾胃生理功能、病理特点探究脾胃与脑相关性,为脾胃论治脑病提供更多思路。脾胃疏中焦、化精微、升清降浊和统血的功能正常可保持脑髓的充盈、痰湿的运化、元神的清明及血液在脉内正常运行。若脾胃受损也可导致脑病的发生。当部分脑病发生时,从调理中焦脾胃角度,脾失健运、痰蒙脑窍者需以补脾疏肝,化痰祛瘀;生化无权、无以养脑者需温补肾脾,填精益髓;脾失统摄、瘀阻脑络者需补益心脾,益气活血。鉴于脾脑相关性理论的不断完善,中医脑病注重节制饮食,调理中焦脾胃,以脾胃论治脑病的思路为中医治疗脑病提供一种行之有效、规范系统方法。临床采用辨证论治的原则运用归脾汤、益气聪明汤、导痰汤、补阳还五汤等方剂加减运用治疗不寐、中风等部分脑病,来研究从脾胃论治部分脑病的可行性,期望为中医部分脑病的防治提供更多思路。According to TCM,the pathogenesis of encephalopathy is complex and changeable.Wind and fire is the cause,and there are more critical and severe cases.There are great differences among individuals and the disease changes rapidly,which endangers the lives of patients in severe cases.Treatment based on syndrome differentiation is needed in diagnosis andA treatment.According to the correlation theory of spleen and stomach and brain,combined with the physiological function and pathological characteristics of spleen and stomach,explore the correlation between spleen and stomach and brain,and provide more ideas for the treatment of encephalopathy by spleen and stomach.The normal function of the spleen and stomach to drain the middle Jiao(焦),purify refined nutritious substances,raise the Qing(清)to reduce the turbidities and regulate the blood can keep the brain full,the phlegm and dampness transport,the pure and clear of the primordial Shen(神)and the blood running normally in the pulse.If the spleen and stomach are damaged,it can also lead to the occurrence of encephalopathy.When part of encephalopathy occurs,from the perspective of regulating the spleen and stomach,the spleen loses health and the phlegm obscurates the brain is needed to replenish the spleen and ease the liver,remove phlegm and remove blood stasis.Unable to produce and transform,without nourishing the brain need warm kidney and spleen,tonify kidney Yin(阴)and nourish the bone marrow,Spleen loss of unification,blood stasis of the brain need to tonify the spleen,Qi(气)and blood circulation.At present,the theory of correlation between spleen and brain has been continuously improved.TCM treatment of encephalopathy pays attention to controlling the diet of patients,recuperating the spleen and stomach,and the idea of treating encephalopathy with the theory of spleen and stomach provides an effective and standardized systematic method for the treatment of encephalopathy.Clinically,the principle of syndrome differentiation is applied in the treatment of
