自拟清热化痰方治疗耐碳青霉烯鲍曼不动杆菌肺炎的临床研究

摘　　要：目的:观察自拟清热化痰方治疗耐碳青霉烯鲍曼不动杆菌(Carbapenem Resistant Acinetobacter Baumannii,CR-AB)肺炎的临床效果。方法:根据样本量估算选择黄冈市中医医院重症医学科CR-AB肺炎患者87例,采用随机数字表法均分为对照组(43例)和治疗组(44例);对照组采用常规西药进行治疗,治疗组在对照组基础上加用自拟清热化痰方治疗。观察并比较两组患者治疗前后总有效率、中医证候积分、急性生理与慢性健康评分表II(Acute Physiology and Chronic Health Evaluation II,APACHE II)等指标。结果:两组患者治疗前一般资料与病情,差异均无统计学意义。治疗后与治疗前比较,两组中医证候积分、APACHE II评分、C反应蛋白(C-reactive Protein,CRP)、降钙素原(Procalcitonin,PCT)均明显下降,氧合指数均明显改善,差异具有统计学意义(P<0.05)。治疗后,与对照组比较,治疗组总有效率明显提高,中医证候积分、APACHE II评分、CRP均明显下降,氧合指数明显改善,差异具有统计学意义(P<0.05);治疗组治疗后PCT与对照组相比差异无统计学意义(P>0.05)。结论:自拟清热化痰方能明显提高CR-AB肺炎患者的有效率,降低患者中医证候积分、APACHE II评分、CRP,改善氧合指数。Objective:To observe the clinical effect of the self-made Qingre Huatan decoction(自拟清热化痰方)on carbapenem resistant acinetobacter baumannii pneumonia.Methods:According to the sample size estimation,we selected 84 patients that met our requirement from ICU of Huanggang TCM Hospital,and divided them into two groups randomly.The control group was treated with conventional western medicine,and the treatment group was treated with the self-made Qingre Huatan decoction on theO basis of the control group.The total effective rate,TCM syndrome score,APACHE II score and other indexes were observed and compared between the two groups.Results:Before treatment,the general data and medical condition between two groups was no significant difference.After treatment,compared with before treatment,TCM syndrome score,APACHE II score,CRP,PCT were significantly decreased and oxygenation index was significantly improved in both groups(P<0.05).After treatment,compared with the control group,the total effective rate in the treatment group was significantly improved,TCM syndrome score,APACHE II score,CRP,PCT were significantly decreased and oxygenation index was significantly improved(P<0.05),there was no difference in PCT between the two groups(P>0.05).Conclusion:The self-made Qingre Huatan decoction can significantly improve the total effective rate of carbapenem resistant acinetobacter baumannii pneumonia,lower the TCM syndrome score,APACHE II score,CRP and ameliorating oxygenation index.
