刘良徛教授基于肺上通咽喉理论从咽喉论治咳嗽经验

摘　　要：咳嗽病因繁多且复杂,而难于针对性治疗。其病机虽总属“肺失宣降,肺气上逆”,但常因为针对病机治疗忽于病因解除,从而治标不治本,导致咳嗽迁延不愈。江西中医药大学附属医院刘良徛教授从医30余载,对于肺系疾病的治疗经验丰富,其临床实践发现咳嗽之因常责于咽喉不利。其依据“咽喉与肺,生理相通,病理相关”与“咽喉经络循行”提出肺上通咽喉理论,据此指出从咽喉论治咳嗽,并且治疗咳嗽时尤其重视咽喉的审察与调理。刘教授依据咽喉为肺之门户,温邪上受而首先犯之,以及燥热易伤及肺脏的生理特点,指出治疗咽喉时宜以“清利”为主,治疗肺则宜“温而不燥”。从咽喉论治咳嗽原则包括:其一,“平调寒热,调和阴阳”为总纲领,以此达到恢复脏腑正气与维系脏腑功能的目的;其二,“补虚泻实,标本兼治”,治疗咳嗽时勿忘治其本;其三,“重视咳因,寻其根本”,不得“见咳止咳”,以免闭门留寇而导致咳嗽难愈。文案介绍刘教授运用寒热并用之法,以银翘马勃散与温肺煎为主方而加味治疗咳嗽验案一则,以期为临床拓展思路。The etiology of cough is diverse and complex,and it is difficult to treat.Although the pathogenesis is always lung loss of ventilation,lung-Qi(气)is reversed,it is often delayed because of the treatment for the pathogenesis and neglect of the cause of the disease.Professor LIU Liangji,from the Affiliated Hospital of Jiangxi University of Traditional Chinese Medicine,hasT been a doctor for more than 30 years and has rich experience in the treatment of lung diseases.His clinical practice found that the cause of cough is often attributed to the adverse effect of the throat.Based on the theory that the throat and lung are physiologically connected and pathologically related and the circulation of the meridians of the throat,he put forward the theory that the lung is connected with the throat.Based on this theory,he pointed out that the treatment of cough should be based on the throat,and special attention should be paid to the examination and conditioning of the throat.Based on the fact that the throat is the gateway to the lung,the warm pathogens are the first to be attacked,and the physiological characteristics of dry heat easily damage the lung,Professor LIU points out that the treatment of the throat should be focused on clearing the heat,while the treatment of the lung should be warm but not dry.This paper introduces professor LIU’s experience in treating cough with the Yinqiao Mabo powder(银翘马勃散)and the Wenfei decoction(温肺散)based on the method of combination of cold and heat,in order to expand the clinical thinking.
