再谈“细辛不过钱”

摘　　要：“细辛不过钱”的说法由来已久,然而仍存在争议,致使临床细辛用量受到一定的制约。文章从文献源流、细辛用量、剂型及给药方式4个方面再谈“细辛不过钱”,得到结论:“细辛不过钱”的确切说法为《本草别说》之“细辛若单用末,不可过半钱匕”;“钱匕”是量药器,“钱”是质量单位,“半钱匕”约合0.3 g,而“一钱”约合3 g;《本草别说》中的“细辛末”可能是一种粒度较为细小的散剂,而非“粗末”;给药方式很可能是外用鼻吹法,而非内服。综合分析可知单用细辛末可因剂量较大、粉末剂型及外用鼻吹法等多重因素,导致“气闷塞,不通者死”的症状,非后世“细辛不过钱”这一片面认识。The saying that‘Asarum’s dosage can not exceed one Qian’has a long history,but there is still controversy,resulting in certain restrictions on the dosage of Asarum in clinical practice.From four aspects:the literature source,Asarum dosage,dosage form and the mode of administration,the article deeply discusses‘Asarum’s dosage can not exceed one Qian’,and the conclusions are as follows:The exact saying of‘Asarum’s dosage can not exceed one Qian’is in Bencao Bieshuo,‘if Asarum is used alone as a powder,it should not exceed half of a coin-spoon’.‘Coin-spoon’is a medicine measuring device,‘Qian’is a unit of mass.‘Half of a coin-spoon’is about 0.3 g,and‘one Qian’is about 3 g.Asarum powder in Bencao Bieshuo may be a powder with relatively small particle size,rather than‘coarse powder’.The mode of administration is likely to be topical nasal insufflation rather than oral administration.Comprehensive analysis shows that the single use of Asarum powder can lead to the symptoms of‘stuffiness,died from suffocation’due to multiple factors such as large doses,powder dosage form and external nasal blowing,which is not the one-sided understanding of‘Asarum’s dosage can not exceed one Qian’in later generations.
