摘 要:目的:探究血清血管内皮生长因子(VEGF)及趋化因子CXC配体12(CXCL12)对2型糖尿病(T2DM)并发微血管病变的预测价值。方法:选取单纯T2DM患者90例为单纯T2DM组(A组),选取T2DM并发微血管病变患者90例为T2DM并发微血管病变(B组),同时体检健康者90例为对照组(C组)。比较三组血压、糖脂及肾功能指标水平,以及血清VEGF、CXCL12水平。分析T2DM并发微血管病变的危险因素。分析血清VEGF、CXCL12对T2DM并发微血管病变的预测价值。结果:与C组比较,A组和B组患者收缩压(SBP)、舒张压(DBP)、总胆固醇(TC)、高密度脂蛋白胆固醇(HDL-C)、甘油三酯(TG)、低密度脂蛋白胆固醇(LDL-C)、空腹血糖(FBG)、血肌酐(Scr)、血尿素氮(BUN)、VEGF和CXCL12水平升高(均P<0.05)。与A组比较,B组上述指标水平升高(均P<0.05)。Logistic回归分析结果显示,TC、FBG、Scr、VEGF、CXCL12是T2DM并发微血管病变的独立危险因素(均P<0.05)。相关性分析表明,VEGF、CXCL12与TC、FBG、Scr呈正相关(均P<0.05)。受试者工作特征(ROC)曲线分析显示,VEGF、CXCL12对T2DM并发微血管病变均有一定的预测价值,两者联合的预测价值更高(均P<0.05)。结论:血清VEGF、CXCL12在单纯T2DM和T2DM并发微血管病变患者中水平均升高,且T2DM并发微血管病变患者中水平更高。两者对T2DM并发微血管病变有一定预测价值,且两者联合价值更高。Objective:To investigate the predictive value of serum vascular endothelial growth factor(VEGF)and chemokine CXC ligand 12(CXCL12)for type 2 diabetes mellitus(T2DM)complicated with microangiopathy.Methods:A total of 90 patients with T2DM were selected as T2DM group(group A),90 patients with T2DM complicated with microangiopathy were selected as T2DM complicated with microangiopathy group(group B),and 90 healthy subjects were selected as control group(group C).The levels of blood pressure,indexes of glucose,lipid and renal function,and serum VEGF and CXCL12 were compared among the three groups.The risk factors of T2DM complicated with microangiopathy,and the predictive value of serum VEGF and CXCL12 for T2DM complicated with microangiopathy was analyzed.Results:Compared with group C,the levels of SBP,DBP,TC,HDL-C,TG,LDL-C,FBG,Scr,BUN,VEGF and CXCL12 increased in group A and group B(all P<0.05).Compared with group A,the levels of the above indicators in group B were increased(all P<0.05).Logistic regression analysis showed that TC,FBG,Scr,VEGF and CXCL12 were independent risk factors for T2DM complicated with microangiopathy(all P<0.05).Correlation analysis showed that VEGF and CXCL12 were positively correlated with TC,FBG and Scr(all P<0.05).ROC curve analysis showed that both VEGF and CXCL12 had a certain predictive value for T2DM with microangiopathy,and the combination of the two had a higher predictive value(all P<0.05).Conclusion:The levels of serum VEGF and CXCL12 are increased in T2DM patients with or without microangiopathy,and the levels are higher in T2DM patients with microangiopathy.Both of them have a certain predictive value for T2DM complicated with microangiopathy,and the combination of the two has a higher value.
