不稳定骨盆骨折合并髋臼骨折手术治疗效果

摘　　要：目的 探讨不同手术方法对不稳定骨盆骨折伴髋臼骨折的疗效。方法 选取2013年10月至2023年10月凌源市中心医院骨外科50例骨盆不稳定伴有髋臼骨折患者为研究对象,分析其临床特点,采用随机数字表法分为两组,分别开展传统髂腹股沟入路手术治疗(甲组)与Stoppa入路手术治疗(乙组),各25例。观察并比较两组手术时间、切口长度、术中出血、术后引流量、骨折愈合时间、骨盆骨折治疗优良率、髋臼骨折治疗优良率、术后并发症发生率。结果 乙组的手术时间、切口长度、术中出血、术后引流量、骨折愈合时间均小于甲组(P <0.05)。乙组骨盆骨折治疗优良率高于甲组(P <0.05)。乙组髋臼骨折治疗优良率高于甲组(P <0.05)。乙组术后并发症发生率低于甲组(P <0.05)。结论 对于不稳定性骨盆骨折伴有髋臼骨折的患者,应用Stoppa术式治疗,疗效明显,不良反应小,有较高的临床应用价值。Objective To investigate the effect of different surgical methods on unstable pelvic fractures with acetabular fractures.Methods 50cases of pelvic instability accompanied by acetabular fracture in the Department of Orthopedics of Lingyuan Central Hospital were analyzed,and their clinical characteristics were analyzed.The data collection time was from October 2013 to October 2023.After data collection,they were divided into two groups,which underwent traditional ilioinguinal approach surgery(group A) and Stoppa approach surgery(group B),respectively,with 25 cases each.The grouping process was determined by random number table method.The operation time,incision length,intraoperative bleeding,postoperative drainage volume,fracture healing time,good and good rate of pelvic fracture,good and good rate of acetabular fracture and postoperative complication rate were observed and compared between the two groups.Results Compared with the surgical indexes,the operative time,incision length,intraoperative bleeding,postoperative drainage volume and fracture healing time in group B were all lower than those in group A,P<0.05.Compared with the good and good rate in the treatment of pelvic fracture,the good and good rate in group B was higher than that in group A,P<0.05.The superior and superior rate of acetabular fracture treatment in group B was higher than that in group A(P<0.05).The incidence of postoperative complications was lower in group B than in group A(P<0.05).Conclusions For the patients with unstable pelvic fracture accompanied by acetabular fracture,Stoppa is effective and has little adverse reaction,and has high clinical application value.
