患者自我报告结局评估工具在全膝关节置换术后疗效评估中的应用研究进展

摘　　要：全膝关节置换术(TKA)是公认的治疗终末期膝骨关节炎的有效治疗手段。患者自我报告结局评估工具(PROM)是术后评估TKA临床疗效的重要方式。2008年以来,越来越多的新兴PROM被报道应用于TKA。第一类PROM用于特定评估人群,包括韩国膝关节评分(KKS)、高活动性评分(HAAS);第二类PROM为特定评估目的而设定,包括遗忘关节评分(FJS)、功能表现/疼痛表现评分(PerF/PerP);第三类是对传统PROMs的优化升级,包括自我管理的患者满意度量表(SAPS)、美国膝关节协会评分(KSS)-2011、膝关节损伤与骨关节炎评分-体力活动功能(KOOS-PS)。这些评价工具从不同的维度评估TKA疗效,极大地丰富了TKA术后疗效评价体系。本文对上述7种新兴PROM的特点及临床应用的研究进展进行综述。Total knee arthroplasty(TKA)is considered as an effective treatment for end-stage knee osteoarthritis(OA).Patient-reported outcome measures(PROMs)plays an important role in the assessments after TKA.Since 2008,the increasing use of PROMs in TKA has been reported.The first type of PROM is used for specific subgroups of patients,including the Korean Knee Score(KKS)and High Activity Score(HAAS).The second type of PROM is set for specific purposes,for example,Forgotten joint score(FJS)and Performance Function/Performance Pain(PerF/PerP).The third type is the optimization and upgrading of traditional PROMs,including Self-Administered Patient Satisfaction Scale(SAPS),Knee Society Score-2011(KSS-2011),and Knee Injury and Osteoarthritis Outcome Score-Physical Function(KOOS-PS).These tools have allowed for the comprehensive outcome evaluation covering distinctive aspects,and greatly enriched postoperative evaluation system in TKA.Here we reviewed the characteristics and clinical application of the aforementioned seven PROMs.
