摘  要:目的:探讨血常规(WBC)、C反应蛋白(CRP)、血清降钙素原(PCT)及肺炎支原体核酸,呼吸道病毒核酸或抗原阳性在呼吸道感染类型中的相关性。方法:收集2019年4月—2021年12月于山东省济宁市妇女儿童医院因呼吸道感染入院的患儿共306例,采集患儿静脉血检测血常规、PCT及CRP等炎症指标并统计分析其相关性。结果:乙型流感病毒、甲型流感病毒、呼吸道合胞病毒、腺病毒间WBC、NE%、CRP比较,差异有统计学意义(P<0.05);不同病毒PCT比较,差异无统计学意义(P>0.05)。支原体核酸阳性与病毒阳性患儿NE%、CRP、PCT3比较,差异无统计学意义(P>0.05);支原体阳性与病毒阳性患儿WBC比较,差异有统计学意义(P<0.05)。抗原阳性与核酸阳性患儿WBC、N%、CRP、PCT比较,差异无统计学意义(P>0.05)。结论:在病毒病原学检测方法中,可根据医院自身的检测条件,抗原检测或核酸检测任选其一即可,当检测结果为阴性时,应用WBC、NE%、CRP检测对于不同病毒及支原体感染的鉴别诊断也具有一定的参考价值。Objective:To investigate the correlation of blood routine(WBC),C-reactive protein(CRP),serum procalcitonin(PCT)and mycoplasma pneumonia nucleic acid,respiratory virus nucleic acid or antigen positive in respiratory tract infection.Methods:A total of 306 children with respiratory tract infection admitted to Women and Children’s of Shandong Province from April 2019 to December 2021 were collected.Venous blood samples were collected to detect blood routine,PCT and CRP,and their correlation was statistically analyzed.Results:There were significant differences in WBC,N%and CRP among influenza B virus,influenza A virus,respiratory syncytial virus and adenovirus(P<0.05).There was no significant difference in PCT among different viruses(P>0.05).There was no significant difference in N%,CRP and PCT3 between mycoplasma nucleic acid positive and virus positive children(P>0.05).There was significant difference in WBC between mycoplasma positive and virus positive children(P<0.05).There was no significant difference in WBC,N%,CRP and PCT between antigen positive and nucleic acid positive children(P>0.05).Conclusion:Among the viral pathogen detection methods,antigen detection or nucleic acid detection can be selected according to the detection conditions of the hospital itself.When the detection results are negative,the application of WBC,NE,CRP detection has certain reference value for the differential diagnosis of different virus and mycoplasma infections.
