利多卡因湿敷联合硝黄解毒汤坐浴对改良痔外剥内扎术后患者的治疗效果

摘　　要：目的:探讨利多卡因湿敷联合硝黄解毒汤坐浴对改良痔外剥内扎术后创面疼痛及愈合的临床应用效果。方法:选取2021年1月—2023年5月在南阳市第二人民医院肛肠科进行改良痔外剥内扎术的92例环状混合痔患者为研究对象,采用1∶1简单随机数字表法分为联合组(利多卡因湿敷联合硝黄解毒汤坐浴治疗,46例)和对照组(单用硝黄解毒汤坐浴治疗,46例)。比较两组肛门疼痛情况、生活质量、术后恢复情况及并发症发生情况。结果:治疗2周后,联合组NRS评分低于对照组,SF-36评分高于对照组,差异有统计学意义(P<0.05);联合组创面愈合时间、水肿消失时间以及首次排便时间均短于对照组,差异有统计学意义(P<0.05);联合组术后并发症发生率与对照组比较,差异无统计学意义(P>0.05)。结论:利多卡因湿敷联合硝黄解毒汤坐浴可缓解改良痔外剥内扎术后患者创面疼痛,促进创面愈合,且不增加并发症发生概率,安全性较高。Objective:To investigate the clinical effect of lidocaine wet compress combined with Xiaohuang Jiedu decoction hip bath on wound pain and healing after modified hemorrhoidectomy.Methods:From January 2021 to May 2023,92 patients with circumferential mixed hemorrhoids who underwent modified external stripping and internal ligation of hemorrhoids in the anorectal department of Nanyang Second People’s Hospital were selected as the research objects.They were divided into combined group(lidocaine wet compress combined with Xiaohuang Jiedu decoction sitz bath treatment,46 patients)and control group(Xiaohuang Jiedu decoction sitz bath treatment alone,46 patients)by 1:1 simple random number table method.The anal pain,quality of life,postoperative recovery and complications were compared between the two groups.Results:After 2 weeks of treatment,the NRS score of the combined group was lower than that of the control group,and the SF-36 score was higher than that of the control group,the differences were statistically significant(P<0.05).The wound healing time,edema disappearance time and first defecation time in the combined group were shorter than those in the control group,the differences were statistically significant(P<0.05)..There was no significant difference in the incidence of postoperative complications between the combined group and the control group(P>0.05).Conclusion:Lidocaine wet compress combined with Xiaohuang Jiedu decoction hip bath can relieve wound pain and promote wound healing in patients after modified hemorrhoid external stripping and internal ligation,without increasing the incidence of complications,with high safety.
