育龄期女性卵巢囊肿蒂扭转复位术后卵巢功能的评估

摘　　要：目的利用三维能量彩超评估育龄期女性卵巢囊肿蒂扭转复位术后卵巢功能。方法选取2021年1月—2022年9月南宁市第一人民医院20例行腹腔镜下卵巢囊肿蒂扭转复位手术治疗的患者,利用三维能量彩超测定术后3个月双侧卵巢血管化指数(VI)、血流指数(FI)、血管化血流指数(VFI),评估术后卵巢功能,比较术后3个月双侧卵巢体积及窦卵泡数。结果术后3个月,健侧VI为(6.04±2.05),FI为(23.41±5.40),VFI为(1.40±0.47),患侧VI为(4.88±2.04),FI为(22.04±7.60),VFI为(1.22±0.43),差异均无统计学意义(t=1.806,0.657,1.238,均P>0.05)。术后3个月健侧窦卵泡数为(8.75±2.05)个,患侧为(4.05±2.11)个,健侧卵巢体积为(10.89±3.62)cm^(3),患侧为(7.31±2.66)cm^(3),差异均无统计学意义(t=-7.139,-3.551,均P>0.05)。结论卵巢囊肿蒂扭转患者保留卵巢的术式安全可行,术后卵巢功能可在一定程度上恢复。Objective To evaluate ovarian function after ovarian cyst torsion reduction in women of childbearing age.Methods Twenty patients who underwent laparoscopic ovarian cyst torsion reduction in the First People's Hospital of Nanning from January 2021 to September 2022 were selected.Ovarian vascularization index(VI),flow index(FI),and vascularized flow index(VFI)at three months after surgery were measured by three-dimensional energy color ultrasound to evaluate ovarian function,bilateral ovarian volumes and numbers of follicles were compared at three months after surgery.Results At three months after surgery,VI on the healthy side was(6.04±2.05),FI on the healthy side was(23.41±5.40),VFI on the healthy side was(1.40±0.47),VI on the affected side was(4.88±2.04),FI on the affected side was(22.04±7.60),VFI on the affected side was(1.22±0.43),there was no statistically significant difference(t=1.806,0.657,1.238,all P>0.05).At three months after surgery,the number of follicles on the healthy side was(8.75±2.05),and the number of follicles on the affected side was(4.05±2.11),ovarian volume on the healthy side was(10.89±3.62)cm^(3),ovarian volume on the affected side was(7.31±2.66)cm^(3),there was no statistically significant difference(t=-7.139,-3.551,all P>0.05).Conclusion It is safe and feasible to preserve ovary in patients with ovarian cyst torsion reduction,and ovarian function can be restored to a certain extent after operation.
