柏子养心丸联合归脾汤加减治疗老年原发性失眠的前瞻性对照研究

摘　　要：目的:探究柏子养心丸联合归脾汤加减治疗老年原发性失眠的临床疗效。方法:选取2020年6月至2023年6月聊城市第四人民医院收治的老年原发性失眠(心脾两虚型)患者100例作为研究对象,按照随机数字表法分为对照组和观察组,每组50例。对照组给予阿普唑仑片治疗,观察组给予柏子养心丸联合归脾汤加减治疗。比较2组疗效、中医症状(多梦易醒、心悸、健忘、头晕目眩、神疲乏力)评分和睡眠结构[总睡眠时间(TST)、睡眠潜伏期(SL)、睡眠效率(SE)、觉醒时间(AT)]的差异。结果:治疗后,观察组的总有效率为94.00%高于对照组的80.00%,差异有统计学意义(P<0.05);2组的各项症状评分、SL、AT均低于治疗前,TST、SE高于治疗前,且观察组症状和睡眠结构改善程度均优于对照组,差异均有统计学意义(均P<0.05)。结论:柏子养心丸联合归脾汤加减对老年原发性失眠患者的疗效显著,能有效改善中医症状,优化睡眠结构。Objective:To explore the clinical efficacy of Baizi Yangxin Pill combined with Modified Guipi Decoction in the treatment of elderly primary insomnia.Methods:From June 2020 to June 2023,100 elderly patients with primary insomnia(heart spleen deficiency syndrome)admitted to our hospital were selected as the study subjects,and were randomly divided into a control group(50 cases)and an observation group(50 cases)using a random number table method.The control group was treated with alprazolam tablets,while the research group was treated with Baizi Yangxin Pill combined with Modified Guipi Decoction.Compare the differences in efficacy,traditional Chinese medicine symptom scores(such as frequent dreaming,palpitations,forgetfulness,dizziness,fatigue,and fatigue),and sleep structure[total sleep time(TST),sleep latency(SL),sleep efficiency(SE),and awakening time(AT)]between the two groups.Results:After treatment,the total effective rate of the study group was 94.00%,which was higher than 80.00%of the control group(P<0.05).The symptom scores,SL,AT of both groups were lower than before treatment,while TST and SE were higher than before treatment.The improvement of symptoms and sleep structure in the observation group was better than that in the control group(P s<0.05).Conclusion:The combination of Baizi Yangxin Pill and Modified Guipi Decoction has a significant therapeutic effect on elderly patients with primary insomnia,which can effectively improve traditional Chinese medicine symptoms and optimize sleep structure.
