可调式口腔矫治器治疗睡眠呼吸暂停综合征的效果分析

摘　　要：目的:探析在睡眠呼吸暂停综合征的治疗中,使用可调式口腔矫治器后,所获得的临床效果。方法:选取2021年11月至2023年1月三明市泰宁县总医院收治的睡眠呼吸暂停综合征患者70例作为研究对象,按照随机数字表法分为对照组和观察组,每组35例。对照组佩戴普通下颌前移类矫治器,观察组佩戴可调式口腔矫治器。在连续佩戴3个月后,就2组患者的治疗效果、临床相关指标、上气道的结构、睡眠的质量等展开比较。结果:观察组的治疗总有效率、血氧饱和度等指标的水平高于对照组;呼吸暂停指数、呼吸暂停低通气指数、匹兹堡睡眠质量指数(PSQI)评分的结果则低于对照组,单次呼吸暂停持续时间比对照组更短,腭咽段、舌咽段截面积大于对照组,差异均有统计学意义(均P<0.05)。结论:对睡眠呼吸暂停综合征患者佩戴可调式口腔矫治器具有显著的治疗效果,能有效地改善气道的结构,缩短呼吸暂停的时间,降低呼吸暂停的频次,进而提升患者的睡眠质量,建议临床推广。Objective:To explore the clinical effect of adjustable oral appliance in the treatment of sleep apnea syndrome.Methods:Patients with sleep apnea syndrome who came to our hospital from November 2021 to January 2023 were randomly selected for the study,and 70 patients were divided into 2 groups according to different treatment methods.Thirty-five patients in the control group were treated with ordinary mandibular advance appliance,and 35 patients in the observation group were treated with adjustable oral appliance.After 3 months of continuous treatment,the therapeutic effect,clinically relevant indicators,upper airway structure and sleep quality of the two groups were compared.Results:The total effective rate and blood oxygen saturation of the study group were higher than those of the control group,the apnea index,apnea hypopnea index and Pittsburgh Sleep Quality Index(PSQI)score were lower than those of the control group,the duration of single apnea was shorter than that of the control group,and the cross-sectional area of palatopharyngeal segment and glossopharyngeal segment was larger than that of the control group,with statistical significance(P s<0.05).Conclusion:Wearing adjustable oral appliance has significant therapeutic effect on patients with sleep apnea syndrome,which can effectively improve the structure of airway,shorten the time of apnea,reduce the frequency of apnea,and thus improve the sleep quality of patients,and it is recommended to promote clinical use.
