摘  要:目的评估定量MRI技术T2 mapping及mDixonQuant在早期椎间盘退变诊断中的潜在临床价值。方法使用3.0TMR对79例行腰椎MRI检查的患者进行T2WI、T2 mapping、mDixonQuant成像,同时记录患者腰痛情况。统计分析分级为PfirrmannⅠ级和PfirrmannⅡ级的椎间盘(其中Ⅰ级76个,Ⅱ级87个)椎体(V)、髓核(NP)的脂肪分数(FF)值、T2 mapping(map)值的差异。对有意义的参数进行受试者工作特征(ROC)曲线分析。结果V-FF与椎间盘退行性病变呈轻度正相关,NP-FF和NP-map值与病变呈轻度负相关。在2组之间,V-FF(P<0.001)、NP-FF(P=0.005)和NP-map(P<0.001)统计学差异显著;不同节段椎间盘分别比较时,部分测量值存在统计学差异。结论V-FF、NP-FF和NP-map与椎间盘退变存在联系,T2 mapping及mDixonQuant具有潜在的价值,可以定量地评估早期椎间盘退变并帮助诊断。Objective To evaluate the potential clinical value of T_(2) mapping and mDixon Quant in the diagnosis of early intervertebral disc degeneration.Methods A total of 79 volunteers who underwent lumbar MRI examination were enrolled.All subjects were examined for 3.oT MR with T_(2) WI,T_(2),mapping,and mDixon Quant while recording the condition of low back pain.The differences between T,mapping(map)value and fat fraction(FF)values of the vertebral(V)and nucleus pulposus(NP)within the Pfirrmann I and Pfirrmann II intervertebral disc(grade I 76,grade II 87)were statistically analyzed.Receiver operating characteristic(ROC)curve analyses were performed for meaningful parameters.Results V-FF showed a mild positive correlation with degenerative intervertebral disc lesions,and NP-FF and NP-map values showed a mild negative correlation with lesions.There were statistically significant dfferences between the two groups in V-FF(P<0.001),NP-FF(P=0.005),and NP-map(P<0.001).Some measurements had statistically significant differences when different intervertebral disc segments were compared.Conclusion V-FF,NP-FF,and NP-map are associated with intervertebral disc degeneration.T,mapping and mDixon Quant are potentially valuable as diagnostic tools to quantitatively assess early intervertebral disc degeneration and help diagnose.
