摘  要:目的定量测量CT图像中Ⅱ型副舟骨(AN)形态学参数特征,分析痛性Ⅱ型AN引起足部力学变化情况。方法回顾性分析含Ⅱ型AN足51例CT图像资料;根据足内侧是否疼痛定义2组,痛性Ⅱ型AN组(病例组)及非痛性Ⅱ型AN组(对照组),测量数据包括舟骨与AN内侧关节间隙(MeJS)、中间关节间隙(MiJS)、外侧关节间隙(LJS),距骨-AN距离(TAND),AN最大径(MaD)、最小径(MiD),并应用独立样本t检验测量相关数据差异。结果病例组MeJS、MiJS、LJS与对照组比较,痛性Ⅱ型AN关节间隙加宽,差异有统计学意义(P<0.05);病例组TAND、MaD、MiD与对照组比较,TAND、AN大小无明显区别,差异无统计学意义(P>0.05)。结论痛性Ⅱ型AN足胫骨后肌腱(PTT)附着于AN,破坏足部受力均衡,导致舟骨、AN间距离加宽。Objective To quantitatively measure the morphological parameters of type I accessory navicular(AN)in CT images and analyze the mechanical changes in the foot caused by painful type II AN.Methods The CT images of 51 patients with type II AN were analyzed retrospectively;According to whether the medial foot was painful,we divided the data into two groups,including the pain type II AN group(case group)and the non-pain type II AN group(control group).The measured data included the navicular and medial joint space(MeJS),middle joint space(MiJS),lateral joint space(LJS),talar-accessory navicular distance(TAND),the maximum diameter(MaD)and minimum diameter(MiD)of AN,and the difference of the relevant data using independent sample t-test was measured.Results Compared with the control group,the MeJS,MijJS,and LJS in the case group showed that the joint space of painful type II AN was widened,and there was a significant difference(P<o.05).Compared with the control group,the TAND,MaD,and MiD of the case group had no significant change,and there was no significant difference(P>0.05).Conclusion In painful type II AN foot,the posterior tibial tendon(PTT)is attached to AN,which destroys the balance of force on the foot,resulting in the widening of the distance between the navicular and AN.
