摘  要:新生儿休克是新生儿重症监护病房患者住院和死亡的主要原因之一。根据血流动力学分型将休克分为四种类型,不同原因的休克在同一个患者中可能会同时出现并可能互相转换,因此,快速明确休克类型对休克治疗至关重要。床旁超声技术作为一种对各重症单元可视化动态检查的评估手段,在成人重症监护领域必不可少。近年来,重症超声技术在新生儿科的应用越来越广泛,床旁即时超声(POCUS)技术可对心肺氧输送器官进行实时动态评估,可明确休克的病理生理机制。本文结合国内外床旁超声的发展现状和新生儿休克特点,阐述POCUS在新生儿休克及治疗中的应用价值。Neonatal shock is one of the main reasons of hospitalization and death in neonatal intensive care units.According to the hemodynamic classification,shock is divided into four different types,and different causes of shock may occur simultaneously and switch to each other in the same patient.Therefore,rapid identification of shock types is crucial for shock treatment.Bedside ultrasound technology,as a visual dynamic examination and evaluation method for various critical care units,is essential in the field of adult intensive care.In recent years,the application of critical ultrasound technology in neonatal pediatrics has become increasingly widespread.Point-of-care ultrasound(POCUS) technology can conduct real-time dynamic evaluation of oxygen delivery organs(heart and lung),clarifying the pathophysiological mechanisms of shock.This article combines with the current development status of bedside ultrasound at home and abroad and the characteristics of neonatal shock,and elaborates on the application value of critical care ultrasound in neonatal shock and treatment.
