组合式外固定架治疗高危老年肱骨近端骨折患者术后凝血功能改变与年龄相关性分析

摘　　要：目的探究组合式外固定架治疗高危老年肱骨近端骨折术后患者凝血功能与年龄的相关性。方法回顾性分析2020年1月至2021年12月于河北省中医院骨伤科行组合式外固定架固定手术治疗的70例高危老年肱骨近端骨折患者的临床资料,其中男25例,女45例;60~69岁组22例,70~79岁组28例,80~89岁组18例,90~99岁组2例。比较4组患者术前1 d,术后1、3、7、14 d凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)、凝血酶时间(TT)、纤维蛋白原(FIB)、D-二聚体各指标变化情况。结果4组患者术前1 d,术后1、3、7、14 d PT、APTT、TT、FIB、D-D差异无统计学意义(P>0.05)。术后70例患者均未出现上肢深静脉血栓。组合式外固定架固定术治疗高危老年肱骨近端骨折手术前后患者凝血四项、D-二聚体指标变化与年龄的相关性较小。结论组合式外固定架固定术治疗高危老年肱骨近端骨折术后患者凝血功能不受年龄影响而改变,深静脉血栓的发生率低。Objective To identify the correlation of changes in coagulation function with the age of high-risk elderly patients with proximal humerus fractures treated with a combined external fixator.Methods The clinical data of 70 high-risk elderly patients with proximal humerus fractures surgically treated with a combined external fixator in the Department of Orthopedics and Traumatology,Hebei Province Hospital of TCM from January 2020 to December 2021 were retrospectively analyzed,including 25 males and 45 females.There were 22 cases in the 60-69 years old group,28 cases in the 70-79 years old group,18 cases in the 80-89 years old group,and 2 cases in the 90-99 years old group.Prothrombin time(PT),activated partial thromboplastin time(APTT),thrombin time(TT),fibrinogen(FIB)and D-dimer(D-D)were compared at 1 day preoperatively,and 1,3,7 and 14 days postoperatively.Results There was no significant differences in PT,APTT,TT,FIB,and D-D at 1 day preoperatively,and 1,3,7 and 14 days postoperatively among the 4 groups(P>0.05).None of the 70 patients developed upper extremity deep vein thrombosis after operation.Surgical treatment of combination external fixator fixation for elderly patients with high-risk proximal humerus fractures before and after surgery had little correlation between the changes of four coagulation indices and D-D with age.Conclusion The coagulation function of elderly patients with high-risk proximal humerus fractures treated with combined external fixator is not correlated with age,and the incidence of deep vein thrombosis is low.
