高通量血液透析对维持性血液透析患者高同型半胱氨酸血症和左心室结构的影响分析

摘　　要：目的:评估高通量血液透析(HFHD)和低通量血液透析(LFHD)在治疗维持性血液透析患者中的效果及对同型半胱氨酸水平和左心室结构的影响。方法:选取2021年5月—2022年6月肇庆市第二人民医院肾内科收治的维持性血液透析(MHD)患者50例作为研究对象,根据治疗方式将患者分为观察组和对照组,各25例。观察组给予HFHD,对照组给予LFHD。比较同型半胱氨酸(Hcy)水平及左心室结构改善情况。结果:治疗前,两组Hcy水平比较,差异无统计学意义(P>0.05);治疗后,两组Hcy水平均降低,且观察组低于对照组,差异有统计学意义(P<0.001)。治疗前,两组左室舒张末内径、LVMI、左房前后径、肺动脉根部内径、室间隔厚度、左室射血分数比较,差异无统计学意义(P>0.05)。治疗后,两组左室舒张末内径、LVMI、左房前后径、肺动脉根部内径小于治疗前,且观察组小于对照组;治疗后,两组左室射血分数大于治疗前,且观察组大于对照组,差异均有统计学意义(P<0.05)。两组室间隔厚度比较,差异无统计学意义(P>0.05)。结论:HFHD对于MHD患者的心血管健康具有积极的影响,相比于传统的LFHD,HFHD可更有效地降低Hcy水平,并改善左心室结构。Objective:To evaluate the effect of high-flux hemodialysis(HFHD)and low-flux hemodialysis(LFHD)in the treat⁃ment of maintenance hemodialysis patients and the effect on homocysteine level and left ventricular structure.Methods:50 pa⁃tients with maintenance hemodialysis(MHD)admitted to the Department of Nephrology of the Second People's Hospital of Zhaoq⁃ing City from May 2021 to June 2022 were selected as the study subjects,and the patients were divided into an observation group and a control group of 25 cases each according to the therapy modality.The observation group was given HFHD and the control group was given LFHD.Homocysteine(Hcy)levels and the improvement of left ventricular structure were compared in two groups.Results:Before treatment,Hcy levels of the two groups were compared,and the difference was not statistically significant(P>0.05);after treatment,Hcy levels of the two groups were reduced,and the observation group was lower than the control group,and the difference was statistically significant(P<0.001).Before treatment,the following indexes of patients in two groups were compared:the left ventricular end-diastolic internal diameter,LVMI,left atrial anterior-posterior diameters,pulmonary artery root internal diameter,interventricular septal thickness,and left ventricular ejection fraction,and the difference was not statisti⁃cally significant(P>0.05).After treatment,the left ventricular end-diastolic internal diameter,LVMI,left atrial anteroposterior di⁃ameter,and pulmonary artery root internal diameter of the two groups were smaller than those before treatment,and the observa⁃tion group was smaller than the control group;after treatment,the left ventricular ejection fraction of the two groups was larger than that before treatment,and the observation group was greater than that of the control group,and the differences were statisti⁃cally significant(P<0.05).Comparison of interventricular septal thickness between the two groups,the difference was not statisti⁃cally significant(P>0.05)
