摘　　要：目的:观察腔内心电定位联合钝性分离经外周静脉置入中心静脉导管(PICC)在血液肿瘤患者中的应用效果。方法:选取2021年3月—2023年2月江门市五邑中医院收治的拟行PICC置管的血液肿瘤患者60例为研究对象,随机分为对照组和研究组,各30例。对照组采用传统穿刺方法置管,研究组采用腔内心电定位联合钝性分离PICC置管。比较两组不同穿刺时间疼痛评分、一次性导管尖端位置准确率、穿刺点出血率。结果:研究组穿刺时、穿刺结束时、穿刺后1 min、穿刺后5 min视觉模拟评分均低于对照组,差异有统计学意义(P<0.001);两组一次性导管尖端位置准确率比较,差异无统计学意义(P>0.05);研究组穿刺点出血率低于对照组,差异有统计学意义(P=0.020)。结论:腔内心电定位联合钝性分离PICC可减轻血液肿瘤患者疼痛感,一次性导管尖端位置准确率较高,穿刺点出血率低。Objective:To observe the application effect of intracavitary electro-positioning combined with blunt separation for peripherally inserted central catheter(PICC)in patients with hematologic oncology.Methods:Sixty cases of hematologic oncology patients admitted to Jiangmen Wuyi Hospital of Traditional Chinese Medicine for proposed PICC placement were selected as the study subjects from March 2021 to February 2023,and were randomly divided into the control group and the study group,with thirty cases in each group.The control group was placed by traditional puncture method,and the study group was placed by intracavitary electro-positioning combined with blunt separation of PICC.The pain scores at different puncture time,accuracy of disposable catheter tip position,and bleeding rate at the puncture point were compared between the two groups.Results:The Visual Analog Scores of the study group were lower than those of the control group at the time of puncture,at the end of puncture,1 min after puncture and 5 min after puncture,and the differences were statistically significant(P<0.001);there was no significant difference in the accuracy of tip position between the two groups(P>0.05);the rate of bleeding at the puncture point in the study group was lower than that in the control group,and the difference was statistically significant(P=0.020).Conclusion:Intracavitary electro-positioning combined with blunt detachment of PICC can reduce pain in patients with hematologic oncology,with higher accuracy of disposable catheter tip position and lower puncture point bleeding rate.
