摘 要:目的:探讨心电图联合心脏彩超在左心房增大中的诊断价值。方法:将2019年6月—2022年5月蚌埠医学院第二附属医院收治的左心房增大患者60例作为观察组,另选取非心血管疾病门诊患者55例作为对照组。两组均接受心电图与心脏彩超检查。分别统计两组心电图[最大P波时限(P_(max))、最小P波时限(Pmin)、P波离散度(P_(d))]与心脏彩超[左心房内径(LAD)、左室舒张末期内径(LVEDD)、心室收缩末期心尖四腔心切面的左心房面积(LAA)]指标,运用受试者工作特征(ROC)曲线评估心电图联合心脏彩超诊断左心房增大的价值。结果:观察组P_(max)、P_(d)及LAA高于对照组,Pmin低于对照组,差异有统计学意义(P<0.001)。ROC曲线分析显示,心电图联合心脏彩超诊断左心房增大的曲线下面积为0.974,相较于单独检测更高,且其灵敏度高于单独诊断;心电图与心脏彩超最佳截断值分别为39.59 ms、15.82 mm^(2)。结论:心电图与心脏彩超在左心房增大中均具有较高的诊断价值,且联合应用诊断价值更显著。Objective:To investigate the diagnostic value of electrocardiogram combined with color ultrasound in left atrial en-largement.Methods:Sixty patients with left atrial enlargement admitted to the Second Affiliated Hospital of Bengbu Medical Col-lege from June 2019 to May 2022 were selected as the observation group,and fifty-five non-cardiovascular disease outpatient clinic were selected as the control group.Both groups received electrocardiogram and color ultrasound examination.Electrocardiogram[maximum P wave duration(P_(max)),minimum P wave duration(Pmin),P wave dispersion(P_(d))]and heart color ultrasound[left atrial di-ameter(LAD),left ventricular end-diastolic diameter(LVEDD),and left atrial area(LAA)at the end of ventricular systole]of the two groups were analyzed respectively.Receiver operating characteristic(ROC)curve was used to evaluate the value of electrocar-diogram combined with color ultrasound in the diagnosis of left atrial enlargement.Results:P_(max),P_(d)and LAA in the observation group were higher than those in the control group,and Pmin was lower than those in the control group,with statistical significance(P<0.001);ROC curve analysis showed that the area under the curve of left atrial enlargement diagnosed by electrocardiogram combined with heart color ultrasound was 0.974,which was higher than that by single detection,and its sensitivity was higher than that by single diagnosis;The optimal cut-off values of electrocardiogram and color ultrasound were 39.59 ms and 15.82 mm^(2),re-spectively.Conclusion:Both electrocardiogram and color ultrasound ultrasound have high diagnostic value in left atrial enlarge-ment,and the combined application has more significant diagnostic value.
