寻常痤疮并发面部马拉色菌毛囊炎的临床特征分析

摘　　要：目的:评估寻常痤疮患者中并发面部马拉色菌毛囊炎的患病率;分析其在青少年与成年患者中的临床特征。方法:分析2020年5月—2020年11月我院皮肤科门诊就诊且面部皮损处毛囊内容物马拉色菌免疫荧光检查阳性(出芽孢子≥6个/高倍视野)痤疮患者的临床资料。结果:1 429例痤疮患者中419例(29.32%)马拉色菌阳性,其中145例患者完成随访。与成年相比,青少年患者在皮损疼痛、性别比例及抗生素用药史等差异无统计学意义。青少年患者皮损和瘙痒较成年重(P<0.001)。皮损主要分布于青少年患者的额部和成年患者的下颌。结论:寻常痤疮患者中并发面部马拉色菌毛囊炎的患病率较高,青少年患者的瘙痒症状及面部皮损分布具有明显的临床特征。Objective:To assess the prevalence of facial Malassezia folliculitis in patients with acne vulgaris,and to analyze the differences in clinical characteristics between adolescent and adult patients.Methods:The clinical data from acne patients who visited the dermatology clinic of our hospital from May 2020 to November 2020 were analyzed.All these patients had been tested positive for Malassezia(≥6 budding spores/high magnification field)of the contents of the follicles on the face by fluorescence microscopy examination.Results:Among 1429 acne patients,419 cases(29.32%)were tested positive for Malassezia,of which 145 patients completed follow-up.Compared with adults,adolescent patients showed no significant differences in lesional pain,gender ratio,and history of antibiotic use.Adolescent patients experienced more severe lesions and itching than adults did(P<0.001).Skin lesions in adolescent patients are mainly distributed on the forehead,whereas lesions in adult patients are mainly in the jaw.Conclusions:The prevalence of facial Malassezia folliculitis in patients with acne vulgaris is relatively high.Itching and distribution of lesions are clinical characteristics of adolescent patients.
