摘　　要：目的检索、评价并汇总国内外甲状腺癌患者生育指导的最佳证据,为临床医护人员开展甲状腺癌患者生育期的指导提供循证依据。方法依据证据资源的“6S模型”系统检索各指南网站、数据库及相关网站中有关甲状腺癌患者生育相关的临床决策、指南、专家共识、证据总结、系统评价和Meta分析,检索时限为建库至2023年3月。由2名接受过系统循证护理培训的研究者独立完成文献的质量评价,并结合专业人士的意见,对符合质量标准的文献进行证据提取及汇总。结果共纳入12篇文献,其中1篇临床决策、7篇指南、3篇专家共识、1篇系统评价。汇总了包括生育安全、生育咨询、生育时机、妊娠期用药指导及监测、产后指导、健康教育6个方面的20条最佳证据。结论本研究的最佳证据制作过程科学、严谨,可为临床医护人员开展甲状腺癌患者生育指导提供参考。建议证据使用者在应用本证据开展生育指导时,应考虑临床实际,并结合患者的意愿和身体具体情况,为患者提供专业的、全面的生育咨询及妊娠期间用药指导,以帮助患者顺利完成生育计划。Objective To retrieve,evaluate and summarize the best evidence of fertility guidance for thyroid cancer patients at home and abroad,so as to provide evidence⁃based basis for clinical medical staff to carry out the guidance of fertility period for thyroid cancer patients.Methods According to the"6S model"of evidence resources,the clinical decisions,guidelines,expert consensus,evidence summary,systematic review and Meta⁃analysis related to fertility in patients with thyroid cancer in guideline websites,databases and related websites were systematically searched.The retrieval time was from the establishment of the database to March 2023.Two researchers who had received systematic evidence⁃based nursing training independently completed the quality evaluation of the literature,and combined with the opinions of professionals,the evidence of the literature that met the quality standards was extracted and summarized.Results A total of 12 articles were included,including 1 clinical decision,7 guidelines,3 expert consensus,and 1 systematic review.20 pieces of best evidence were summarized in six aspects,including fertility safety,fertility counseling,fertility timing,medication guidance and monitoring during pregnancy,postpartum guidance,and health education.Conclusion The best evidence of this study is scientific and rigorous,which can provide reference for clinical medical staff to carry out fertility guidance for thyroid cancer patients.It is suggested that when applying this evidence to carry out fertility guidance,users should consider the clinical practice,combine the wishes and physical conditions of patients,and provide patients with professional and comprehensive fertility consultation and medication guidance during pregnancy,so as to help patients successfully complete the fertility plan.
