摘  要:目的探讨数字化设计及3D打印技术设计制作的肋骨胸壁外固定器应用于临床的可行性及优势。方法选取40例多根多处肋骨骨折患者,按随机数字表法分为实验组和对照组,每组20例。实验组采用3D打印的外固定器治疗,对照组采用切开复位内固定治疗。比较两组患者手术时间、术中出血量、疼痛指数、镇痛药物用量、肺部并发症发生率、术后血气胸恢复情况、术后引流量、拔管时间、肺功能改善情况、平均住院费用以及平均住院日。结果两组一般资料差异无统计学意义;疼痛指数、镇痛药物用量、肺部并发症发生率、术后血气胸治愈率、肺功能改善率两组比较,无统计学差异(P>0.05);实验组的手术时间、术中出血量、术后引流量、拔管时间、平均住院费用以及平均住院日显著优于对照组(P<0.05)。结论通过数字化设计及3D打印技术设计制作的肋骨胸壁外固定器治疗多根多处肋骨骨折创伤小,操作简单,固定牢靠,患者术后恢复快,能减轻疼痛,提高生活质量,减轻经济负担,且可实现个性化、精准化、微创化的要求,实用性强,适宜推广使用。Objective To explore the feasibility and advantages of applying digital design and 3D printing technology to design and print rib chest external fixators for clinical use.Methods Forty patients with multiple rib fractures were selected and divided into an experimental group and a control group by random number table,with 20 cases in each group.The experimental group was treated with a 3D-printed external fixator,while the control group was treated with open reduction and internal fixation.The surgical time,intraoperative blood loss,pain index,analgesic drug usage,incidence of pulmonary complications,postoperative recovery of pneumothorax,drainage volume,extubation time,improvement in lung function,average hospitalization cost,and length of hospital stay between the two groups were compared.Results There was no statistically significant difference in general data between the two groups.No statistically significant difference was observed between the two groups in terms of pain index,analgesic drug usage,incidence of pulmonary complications,postoperative recovery of pneumothorax,or improvement in lung function(P>0.05).The surgical time,intraoperative blood loss,postoperative drainage volume,extubation time,average hospitalization cost,and length of hospital stay in the experimental group were significantly better than those in the control group(P<0.05).Conclusion Through digital design and 3D printing technology,a rib chest external fixator can be de-signed and printed for the treatment of multiple fractures of multiple ribs,which has the advantages of mini-mal trauma,simple operation,reliable fixation,fast postoperative recovery,and it can relieve patients'pain,improve their quality of life,and reduce their economic burden.It meets the requirements of person-alization,precision,and minimally invasive treatment,and has strong practicality,making it suitable for promotion and clinical application.
