老年急性脑卒中患者术前家属代理决策现状及影响因素的研究

摘　　要：目的了解老年急性脑卒中患者术前家属代理决策现状并探究其影响因素。方法采用便利抽样法于2020年9月—2022年9月对本院117例老年急性脑卒中患者采取一般资料调查问卷、决策参与量表、决策冲突量表等进行问卷调查,并采用多元回归分析老年急性脑卒中患者术前家属代理决策的影响因素。结果老年急性脑卒中患者术前家属代理决策冲突得分为(40.22±11.05)分,对疾病了解程度比较,差异均无统计学意义(P>0.05);年龄、居住地、家庭月收入、与患者感情状况、以往就医体验、对疾病了解程度、期望决策类型比较,差异均有统计学意义(P<0.05);多元线性回归结果显示家庭月收入、与患者感情状况、文化程度为老年急性脑卒中患者术前家属代理决策的影响因素(P<0.05)。结论老年急性脑卒中患者术前家属代理决策冲突受到多种因素的影响,其中医护人员在其中起主要作用,应评估患者的决策现状制定针对性的干预方案从而改善患者家属代理的决策现状。Objective To understand current situation of preoperative family decision-making of elderly patients with acute stroke and explore its influencing factors.Methods The paper chose 117 elderly patients with acute stroke from September 2020 to September 2022 with convenience sampling method for questionnaire survey,including general information survey questionnaire,decision participation scale,decision conflict scale,etc,and analyzed influencing factors of preoperative family representative decision-making of elderly patients with acute stroke with multiple regression analysis.Results Scores of preoperative family decision-making conflict of elderly patients with acute stroke were(40.22±11.05),there was no statistically significant difference in disease understanding(P>0.05).There was no statistically significant difference in age,place of residence,monthly family income,emotional status,past medical experience of patients,disease understanding and expected decision-making type(P<0.05).Multiple linear regression showed,monthly family income,emotional status and educational level of patients was influencing factors of preoperative family decision-making of elderly patients with acute stroke(P<0.05).Conclusion Decision-making conflict of preoperative family members of elderly patients with acute stroke may be influenced by various factors,medical staff plays a major role.It is necessary to evaluate decision-making status of patients and develop targeted intervention plans to improve decision-making status of patient family members.
