俞募配穴联合脐针疗法治疗脑卒中后昼夜节律失调性睡眠-觉醒障碍的临床研究

摘　　要：目的探讨俞募配穴联合脐针疗法治疗脑卒中后昼夜节律失调性睡眠-觉醒障碍(circadian rhythm sleep-wake disorder,CRSWD)的临床疗效。方法选取2020年5月至2022年8月就诊于杭州市中医院的脑卒中后CRSWD患者87例为研究对象,根据随机数字表法将其分为治疗组(n=44)和对照组(n=43)。对照组患者予艾司唑仑片治疗,治疗组患者予俞募配穴联合脐针疗法治疗,观察两组患者治疗前、治疗5d、治疗2周的匹兹堡睡眠质量指数(Pittsburgh sleep quality index,PSQI)和清晨型-夜晚型量表(morningness-eveningness questionnaire,MEQ)评分。结果两组患者的PSQI总分、PSQI-D、PSQI-G评分、MEQ评分以时点因素、时点与组别交互、组别为源的主体间效应比较,差异均有统计学意义(P<0.05);治疗2周后,治疗组患者的PSQI总分、PSQI-C、PSQI-D、PSQI-G评分均显著低于对照组,MEQ评分显著高于对照组(P<0.05)。结论俞募配穴联合脐针疗法治疗脑卒中后CRSWD可显著改善患者的睡眠质量,延长睡眠时间,改善日间功能状态,调整昼夜节律,有良好的临床疗效。Objective To investigate the clinical effect of back-shu points and front-mu points combination combined with umbilical acupuncture on circadian rhythm sleep-wake disorder(CRSWD)after stroke.Methods A total of 87 patients with post-stroke CRSWD treated in Hangzhou Hospital of Traditional Chinese Medicine from May 2020 to August 2022 were selected as the study objects,and they were divided into treatment group(n=44)and control group(n=43)according to random number table method.The control group was treated with estazolam tablets,and the treatment group was treated with back-shu points and front-mu points combination combined with umbilical acupuncture.Pittsburgh sleep quality index(PSQI)and morningness-eveningness questionnaire(MEQ)of two groups were observed before and after 5 days/2 weeks treatment.Results The PSQI total score,PSQI-D,PSQI-G score and MEQ score of two groups were compared by time-point factors,time-point interaction with the group,and inter-subject effects derived from the group,and the differences were statistically significant(P<0.05).After 2 weeks of treatment,the total PSQI score,PSQI-C,PSQI-D and PSQI-G scores in treatment group were significantly lower than those in control group,and the MEQ scores were significantly higher than those in control group(P<0.05).Conclusion Back-shu points and front-mu points combination combined with umbilical acupuncture therapy for CRSWD after stroke can significantly improve patients’sleep quality,prolong sleep time,improve daytime functional status,adjust circadian rhythm,and have good clinical efficacy.
