摘 要:目的探讨多模态超声在子宫内膜息肉(endometrial polyp,EP)诊断中的应用价值。方法回顾性分析2022年12月至2023年4月浙江省立同德医院收治的90例疑诊EP患者的临床资料。所有患者术前均接受经阴道二维超声和多模态超声(包括经阴道二维超声、彩色多普勒、经阴道三维超声、子宫静脉超声造影)检查,以术后病理结果为金标准,对比超声检查的结果,分析经阴道二维超声与多模态超声诊断EP的效能。结果90例患者中病理确诊EP 73例,非EP 17例。经阴道二维超声诊断EP57例,非EP33例,诊断敏感度为61.16%(45/73),特异性为29.41%(5/17),准确度为55.56%(50/90);多模态超声诊断EP 71例,非EP 19例,诊断敏感度为94.52%(69/73),特异性为88.23%(15/17),准确度为93.33%(84/90)。多模态超声诊断EP的敏感度、特异性、准确度均高于经阴道二维超声(P<0.05)。结论多模态超声检查可显著提高EP诊断的准确度。Objective To investigate the application value of multimodal ultrasound in the diagnosis of endometrial polyps(EP).Methods The clinical data of 90 suspected EP patients admitted to Tongde Hospital of Zhejiang Province from December 2022 to April 2023 were retrospectively analyzed.All patients underwent transvaginal two-dimensional ultrasound and multimodal ultrasound(including transvaginal two-dimensional ultrasound,color Doppler,transvaginal three-dimensional ultrasound,uterine venous contrast ultrasound)before surgery.Using the postoperative pathological results as the gold standard,the results of ultrasound examination were compared to analyze the efficacy of transvaginal two-dimensional ultrasound and multimodal ultrasound in the diagnosis of EP.Results Among 90 patients,73 cases were pathologically confirmed with EP and 17 cases were non-EP.There were 57 cases of EP and 33 cases of non-EP diagnosed by transvaginal two-dimensional ultrasonography.The diagnostic sensitivity was 61.16%(45/73),the specificity was 29.41%(5/17),and the accuracy was 55.56%(50/90).Multimodal ultrasonography diagnosed 71 cases of EP and 19 cases of non-EP.The diagnostic sensitivity was 94.52%(69/73),the specificity was 88.23%(15/17)and the accuracy was 93.33%(84/90).The sensitivity,specificity and accuracy of multimodal ultrasound in the diagnosis of EP were higher than those of transvaginal two-dimensional ultrasound(P<0.05).Conclusion Multimodal ultrasonography can significantly improve the accuracy of EP diagnosis.
