益气温阳活血利水法联合西医常规方法治疗慢性心衰合并顽固性水肿临床研究

摘　　要：目的:探讨益气温阳活血利水法联合西医常规方法治疗慢性心衰合并顽固性水肿效果。方法:选取2022年10月—2023年3月在潜山市中医院内科住院的慢性心力衰竭合并顽固性水肿患者共80例,依据随机数字表法进行分组,对照组39例,采用西医常规方法治疗,实验组41例,采用益气温阳、活血利水法联合西医常规方法治疗。统计2组患者临床疗效、心功能、中医症状积分、6min步行试验(6 Min Walking Test,6MWT)、血清氨基末端脑钠肽前体(N-terminal pro-B-type Natriuretic Peptide,NT-proBNP)、24h尿量、生活质量。结果:实验组患者临床有效率95.12%高于对照组患者临床有效率76.92%;实验组患者左心室舒张末期直径水平低于对照组,左心室射血分数高于对照组;实验组患者中医症状积分低于对照组;实验组患者6MWT、24h尿量高于对照组,血清NT-proBNP水平均低于对照组,差异均有统计学意义。结论:益气温阳活血利水法联合西医常规方法治疗慢性心衰合并顽固性水肿具有积极作用,尤其在改善患者症状、心衰、生活质量方面均有优势。观察指标最好要有针对顽固性水肿的,能体现出肿消或小便量增多。Objective:To explore the effect of replenishing temperature and yang,activating blood circulation and diuresis combined with conventional Western medicine methods in the treatment of chronic heart failure combined with refractory edema.Methods:A total of 80 patients with chronic heart failure combined with refractory edema who were hospitalized in the Internal Medicine Department of Qianshan Hospital of Traditional Chinese Medicine from October 2022 to March 2023 were selected and divided into groups according to the random number table method.There were 39 patients in the control group,using conventional Western medicine methods.Treatment:41 cases in the experimental group were treated with methods of replenishing temperature and Yang,activating blood and diuresis combined with conventional Western medicine methods.The clinical efficacy,cardiac function,traditional Chinese medicine symptom scores,6 Min Walking Test(6MWT),and serum N-terminal pro-B-type Natriuretic Peptide(NT-proBNP)of the 2 groups of patients were counted.,24h urine output,quality of life.Results:The clinical effective rate of the patients in the experimental group was 95.12%,which was higher than that of the patients in the control group,which was 76.92%;the LVEDD level of the patients in the experimental group was lower than that of the control group,and the LVEF was higher than that of the control group;the TCM symptom scores of the patients in the experimental group were lower than those of the control group;The 6MWT and 24h urine output of the patients were higher than those of the control group,and the serum NT-proBNP levels were lower than those of the control group,and the differences were statistically significant.Conclusion:The method of replenishing temperature and yang,activating blood circulation and diuresis combined with conventional Western medicine methods has a positive effect in the treatment of chronic heart failure combined with refractory edema,especially in improving patients’symptoms,heart failure and quality of life.Ob
