类风湿关节炎肠道菌群结构特征及其临床意义分析

摘  要:目的分析类风湿关节炎(RA)肠道菌群结构特征及其临床意义。方法选取2020年8月至2021年10月江西中医药高等专科学校附属医院收治的25例类风湿关节炎患者作为观察组,另选取同期25名健康体检者作为对照组。两者均进行类风湿因子(RF)水平、肠道菌群、炎症指标检测,比较两组RF水平、炎症指标[C反应蛋白(CRP)、白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)],分析肠道微生物群与RA炎症指标(CRP、IL-6、TNF-α)的相关性。结果观察组RF水平高于对照组,双歧杆菌、类杆菌数量均少于对照组,CRP、IL-6、TNF-α水平均低于对照组,差异有统计学意义(P<0.05);两组乳杆菌、梭菌、肠杆菌、肠球菌数量比较差异无统计学意义。Spearman相关性分析结果显示,类杆菌、双歧杆菌与CRP、IL-6、TNF-α均呈负相关(r<0,P<0.05)。结论RA与类杆菌、双歧杆菌呈负相关,为预防及治疗RA提供新思路。Objective To explore and analyze the structural characteristics of intestinal flora in rheumatoid arthritis(RA)and its clinical significance.Methods A total of 25 RA patients admitted to the Affiliated Hospital of Jiangxi College of Traditional Chinese Medicine from August 2020 to October 2021 were selected as the observation group,and 25 healthy subjects in the same period were selected as the control group.The two groups were tested for rheumatoid factor(RF)levels,intestinal flora,and inflammatory indicators,the RF levels and inflammatory indicators(C-reactive protein[CRP],interleukin-6[IL-6],tumor necrosis factor-α[TNF-α])were compared between the two groups,and the correlation between intestinal flora and RA inflammation indicators(CRP,IL-6,TNF-α)was analyzed.Results The RF levels in the observation group was higher than that in the control group,and the number of Bifidobacteria and Bacteroides were less than those in the control group,the levels of CRP,IL-6 and TNF-αin the observation group were lower than those in the control group,the differences were statistically significant(P<0.05).There were no significant differences in the number of Lactobacillus,Clostridium,Enterobacter and Enterococcus between the two groups.Spearman correlation analysis showed that Bifidobacteria and Bacteroides were negatively correlated with CRP,IL-6 and TNF-α(r<0,P<0.05).Conclusion RA is negatively correlated with Bacteroides and Bifidobacteria,which provides a new idea for the prevention and treatment of RA.
