摘 要:目的探讨凝血功能异常早产儿需血浆输注的影响因素。方法回顾性分析2017年2月至2020年12月厦门市中医院新生儿重症监护室(NICU)收治的80例凝血功能异常早产儿的临床资料。所有患儿均肌注维生素K1,若凝血功能未见好转,需输注新鲜冰冻血浆。分析患儿血浆输注情况,采用单因素及多因素Logistic回归分析凝血功能异常患儿需输注血浆的影响因素。结果80例凝血功能异常患儿中,需输注血浆39例,未输注血浆41例。单因素结果分析显示,输注血浆患儿胎龄≤32周、宫内窘迫、酸中毒及呼吸衰竭比例均高于较未输注血浆患儿,差异有统计学意义(P<0.05);多因素Logistic回归分析结果显示,胎龄≤32周、宫内窘迫是影响凝血功能异常患儿需输注血浆治疗的独立危险因素(P<0.05)。结论早产儿胎龄越小,宫内窘迫越严重,对其体内凝血相关因子分泌水平的影响也越深,血浆输注治疗可有利于增强维生素K1对患儿病情的改善效果。Objective To investigate the influencing factors of plasma transfusion in premature infants with abnormal coagulation function.Methods The clinical data of 80 premature infants with abnormal coagulation function admitted to the neonatal intensive care unit(NICU)of Xiamen Hospital of Traditional Chinese Medicine from February 2017 to December 2020 were retrospectively analyzed.All children were intramuscularly injected with vitamin K1,if the coagulation function did not improve,fresh frozen plasma should be infused.The plasma infusion situation of the children was analyzed,Univariate and multivariate Logistic regression analysis was used to analyze the influencing factors of plasma infusion in premature infants with abnormal coagulation function.Results Among 80 cases with coagulation dysfunction,39 cases needed plasma infusion and 41 cases did not receive plasma infusion.Univariate analysis showed that the proportion of gestational age≤32 weeks,intrauterine distress,acidosis and respiratory failure in children with plasma transfusion were higher than those in children without plasma infusion,and the differences were statistically significant(P<0.05).Multivariate Logistic regression analysis showed that gestational age≤32 weeks and intrauterine distress were independent risk factors for plasma infusion in children with abnormal coagulation function(P<0.05).Conclusion The smaller the gestational age of premature infants,the more severe the intrauterine distress,and the deeper the influence on the secretion level of coagulation-related factors in the body,plasma infusion therapy can enhance the improvement effect of vitamin K1 on the condition of children.
