摘 要:目的观察基于多模态神经影像的虚拟现实技术在微血管减压手术教学中的应用效果,探讨其应用的可行性和有效性。方法选择2018年6月—2022年12月在本院功能神经外科病区进行规范化培训的40名住院医师作为研究对象,随机分为虚拟现实互动教学组和传统教学组两组,每组各20名。对于虚拟现实互动教学组,在术前对三叉神经痛和面肌痉挛患者构建出血管神经压迫的虚拟现实模型,通过模拟手术体位、手术入路三维立体观察虚拟现实的模拟,并结合微血管减压手术中手术显微镜下的讲解,让学生判断有无血管神经压迫以及分析责任血管与神经的空间解剖位置关系。在教学阶段结束后对两组学员进行微血管减压手术相应临床理论的知识考核,内容以教学内容为主。同时两组学员在教学阶段结束后对授课方式的满意度、创新性认可度以调查问卷的形式进行反馈。结果虚拟现实互动教学组学生理论考核成绩为(88.95±2.80)分,传统教学组理论考核成绩为(81.65±5.84)分,虚拟现实互动教学组学生理论考核成绩明显优于传统教学组(P<0.05)。虚拟现实互动教学组对其授课方式的满意度、创新性认可度高于传统教学组,同时虚拟现实结合的教学方法相较与传统教学方法而言更容易提升初次接触功能神经外科规培住院医师的兴趣,在学习上带给他们更大的帮助,虚拟现实在功能神经外科血管减压术中教学中的应用可以帮助低学生尽快掌握后颅窝血管神经复杂的解剖,建立一个完整的知识结构体系。结论多模态神经影像融合的虚拟现实互动教学方式有助于加深学生对术前影像学、后颅窝神经血管解剖及微血管减压手术治疗过程的认识和理解,能显著提升学生学习兴趣、学习效率及教学内容掌握度。Objective To observe the application effect of virtual reality technology based on multi-modal neuroimaging in microvascular decompression surgery teaching,and explore the applicable feasibility and efficiency of the technology.Methods Forty residents who received standardized training in functional neurosurgery ward of PLA General Hospital from June 2018 to December 2022 were enrolled as study subjects,and they were randomly divided into virtual reality interactive teaching group and traditional teaching control group.For the virtual reality interactive teaching group,a virtual reality model of vascular nerve compression was constructed preoperative for patients suffering trigeminal neuralgia and facial spasm.Through simulating the surgical posture and surgical approach,three-dimensional virtual reality simulation was observed,and combined with the explanation under the surgical microscope during microvascular decompression surgery,students were asked to determine whether there was vascular nerve compression and analyze the spatial anatomical position relationship between the responsible blood vessels and nerves.At the end of the teaching phase,the two groups of students were tested for their knowledge of the corresponding clinical theories of microvascular decompression surgery,with the main content being the teaching content.At the same time,two groups of students gave feedback on their satisfaction with the teaching method and innovative recognition in the form of questionnaires after the teaching stage.Results The theoretical assessment score of students in the virtual reality interactive teaching group was(88.95±2.80),while the theoretical assessment score of traditional teaching group was(81.65±5.84).The theoretical assessment score of students in the virtual reality interactive teaching group was significantly better than that of traditional teaching group(P<0.05).The virtual reality interactive teaching group had higher satisfaction and innovation recognition than the traditional teaching group.Compare
