超声弹性成像评分法与BI-RADS-US分类在乳腺实性肿块良恶性诊断中的临床价值

摘  要:目的探讨临床诊断鉴别乳腺实性肿块良恶性采用超声弹性成像评分法、BI-RADS-US分类的应用价值。方法选择2021年5月—2022年10月本院接诊治疗乳腺实性肿块患者75例进行回顾性分析,分别接受超声弹性成像评分法、BI-RADS-US分类评估,以手术或活检穿刺结果为金标准,分别记录两种评估方法的结果,同时计算超声弹性成像评分法、BI-RADS-US分类及二者联合诊断乳腺实性肿块良恶性的灵敏度、特异性及准确率情况,进行组间比较。结果75例乳腺实性肿块患者通过手术或活检穿刺诊断结果显示,恶性肿块患者共29例,占比38.67%;良性肿块患者共46例,占比61.33%。经计算结果显示,超声弹性成像评分法联合BI-RADS-US分类评估的灵敏度为96.55%、准确率为94.67%,均明显高于单一评估方式(P<0.05)。结论临床鉴别诊断乳腺实性肿块良恶性可采取超声弹性成像评分法联合BI-RADS-US分类法,相比单一评估更具灵敏性及准确率,值得推广。Objective To investigate the clinical diagnosis and differentiation of benign and malignant solid breast masses using ultrasonic elastography scoring method and BI-RADS-US classification.Methods A retrospective analysis was performed on 75 patients with solid breast masses treated in the Second People's Hospital of Xiangcheng District from May 2021 to October 2022.They were evaluated by ultrasound elastography scoring method and BI-RADS-US classification,with the results of surgery or biopsy as the gold standard.The results of the two evaluation methods were recorded,and the sensitivity,specificity and accuracy of ultrasonic elastography scoring,BI-RADS-US classification and their combined diagnosis of benign and malignant breast masses were calculated,and compared between groups.Results Among 75 patients with solid breast mass diagnosed by operation or biopsy puncture,29 cases were malignant,accounting for 38.67%;46 cases of benign mass,accounting for 61.33%.The calculation results showed that ultrasonic elastic imaging scoring method combined the sensitivity and accuracy of BI-RADSUS classification evaluation were 96.55%and 94.67%,both of which were significantly higher than that of a single evaluation method(P<0.05).Conclusion Ultrasound elastography scoring combined with BIRADS-US classification is more sensitive and accurate than a single evaluation in differentiating benign and malignant breast masses.It is worth popularizing.
