不同流速碘佛醇造影剂行CT肺动脉造影的图像质量观察

摘　　要：目的:分析不同流速碘佛醇造影剂行CT肺动脉造影(CTPA)的图像质量。方法:选取2018年1月~2021年8月医院收治的78例肺栓塞患者为观察对象,在院经CTPA检查时,采用350mgI/mL碘佛醇造影剂,按不同造影剂注射速率分为4.0、3.5、3.0mL/s组,比较三组CTPA图像质量,统计肺栓塞检出率。结果:三组间肺动脉主干、上腔静脉、肺静脉CT值及SNR、CNR比较(P>0.05),4.0mL/s组图像主观评分略微高于3.5、3.0mL/s组,伪影评分略微低于3.5、3.0mL/s组,但差异无统计学意义(P>0.05)。三组间肺栓塞检出率比较(P>0.05)。三组患者的辐射情况容积CT剂量指数、剂量长度乘积、有效剂量比较,差异无统计学意义(P>0.05)。三组均未发生急性肾衰竭、过敏、心搏骤停、心动过速等不良反应。结论:采用350mgI/mL碘佛醇造影剂进行CT肺动脉造影,碘注射速率降低可降低肺动脉强化程度,但图像质量可满足临床诊断需求。Objective:To analyze the image quality of CT pulmonary artery angiography(CTPA)at different flow rates.Methods:In 78 pulmonary embolism patients admitted to the hospital from January 2018 to August 2021,350mgⅠ/mL iopiferol contrast agents were divided into 4.0,3.5,3.0mL/s group according to different contrast injection rates,the quality of CTPA images in the three groups was compared,and the detection rate of pulmonary embolism was calculated.Results:CT values of main,VVC and SNR and CNR among the three groups(the subjective score of P>0.05),4.0mL/s group was slightly higher than 3.5,3.0mL/s group,and the artifact score was slightly lower than 3.5,3.0mL/s group,but the difference was not statistically significant(P>0.05).Comparison of the detection rate of pulmonary embolism among the three groups(P>0.05).There was no significant difference in radiation volume CT dose index,dose length product and effective dose among the three groups(P>0.05).There were no adverse reactions such as acute renal failure,allergies,cardiac arrest,or tachycardia in the three groups.Conclusion:CT pulmonary angiography was performed using 350mg I/mL iodine contrast agent.The decrease in iodine injection rate can reduce the degree of pulmonary artery enhancement,but the image quality can meet clinical diagnostic needs.
