摘 要:目的:探究自体髂骨植骨对急诊老年肱骨近端粉碎性骨折患者锁定钢板内固定治疗疗效的影响。方法:于2018年9月~2022年9月本院急诊收治的老年粉碎性骨折患者中选取62例作为研究对象,并按随机数字表法分成对照组(31例)和观察组(31例)。对照组予以锁定钢板内固定治疗,观察组予以锁定钢板内固定+自体髂骨植骨治疗。比较两组疗效、肩关节主动活动度(AROM)、Constant-Murley肩关节功能(CMS)评分和并发症发生率的差异。结果:观察组骨折复位优良率90.32%高于对照组的67.74%(P<0.05);两组术后1年肩关节各运动的AROM、CMS量表各维度评分均高于术前,且观察组高于对照组(P<0.05);随访期间两组并发症发生率无明显差异(P>0.05)。结论:自体髂骨植骨用于急诊粉碎性骨折患者锁定钢板内固定治疗的效果显著,能有效提高AROM,促进关节功能恢复,且应用较安全。Objective:To explore the effects of autogenous iliac bone graft on the curative effect of locking plate internal fixation in elderly patients with comminuted proximal humerus fracture(PHF).Methods:A total of 62 elderly patients with comminuted PHF admitted to the hospital were enrolled as the research objects between September 2018 and September 2022.According to random number table method,they were divided into control group(31 cases)and observation group(31 cases).The control group was treated with locking plate internal fixation,while observation group was additionally treated with autogenous iliac bone graft.The curative effect,shoulder active range of motion(AROM),Constant-Murley score(CMS)and incidence of complications were compared between the two groups.Results:The good rate of fracture reduction in observation group was higher than that in control group(90.32%vs 67.74%,P<0.05).At 1 year after surgery,scores of AROM and CMS in both groups were increased,which were higher in observation group than control group(P<0.05).There was no significant difference in the incidence of complications between the two groups during follow-up(P>0.05).Conclusion:The curative effect of autogenous iliac bone graft combined with locking plate internal fixation is significant in elderly patients with comminuted PHF,which can effectively improve AROM and promote recovery of joint function,with high safety.
