摘  要:目的探讨盆底肌生物反馈电刺激联合电针俞募穴对脊髓损伤(SCI)神经源性膀胱(NB)患者尿动力学及排尿功能的影响。方法将60例SCI后NB患者随机分为联合组和对照组,每组各30例。对照组采用盆底肌生物反馈电刺激治疗,联合组在此基础上给予电针俞募穴,均治疗8周。比较两组临床疗效、尿动力学及排尿功能变化。结果联合组总效率达86.7%,显著高于对照组63.3%(P<0.05)。两组康复治疗后最大排尿量、最大膀胱量、残余尿量及充盈期逼尿肌压力等各尿动力学指标均显著改善,而联合组的改善程度显著优于对照组(P<0.05)。康复治疗后,两组24 h排尿次数、24 h尿失禁次数及24 h排尿量均明显改善,而联合组各指标均明显优于对照组(P<0.05)。结论生物反馈联合电针俞募穴应用于SCI后NB可改善患者膀胱储尿、排尿功能,促进患者康复。Objective To explore clinical effect of pelvic floor biofeedback myoelectric stimulation combined with electroacupuncture Back-Shu points and Front-Mu points on urodynamics and urination function in patients with neurogenic bladder(NB)after spinal cord injury(SCI).Methods A total of 60 NB patients after SCI were randomly divided into the combination group(n=30)and the control group(n=30).The control group was treated with pelvic floor biofeedback myoelectric stimulation,and the combination group was treated with electroacupuncture at Back-Shu points and Front-Mu points on this basis,all for 8 weeks.Clinical efficacy,urodynamics and urination function were compared between the two groups.Results The total efficiency of the combined group was 86.7%,which was significantly higher than that of the control group(63.3%,P<0.05).After rehabilitation treatment,the maximum urination volume,maximum bladder volume,residual urine volume and detrusor pressure during filling period were significantly improved in the two groups,and the improvement degree in the combined group was significantly better than that in the control group(P<0.05).After rehabilitation treatment,the frequency of 24 h urination,frequency of 24 h urinary incontinence and volume of 24 h urination in both groups were significantly improved,while each index in the combined group was significantly better than that in the control group(P<0.05).Conclusion Biofeedback combined with electroacupuncture Back-Shu points and Front-Mu points for NB after S CI can improve the function of bladder storage and urination,promote the recovery.
