摘 要:目的探究肺间质性病变鉴别诊断中,多层螺旋CT的应用价值。方法选取2021年7月—2023年7月江阴市周庄医院放射科收治的60例疑似肺间质性病变患者,以病理学诊断结果为“金标准”,分析常规CT、多层螺旋CT诊断的应用价值,对上述诊断的肺间质性病变、非肺间质性病变检出率、诊断符合率、灵敏度、特异度、阴性预测值进行分析。结果60例疑似肺间质性病变患者中,病理学诊断结果显示阳性检出50例,多层螺旋CT诊断检出49例、常规CT检出41例,多层螺旋CT诊断检出率高于常规CT检,差异分析有意义(P<0.05),多层螺旋CT与病理学诊断结果差异小(P>0.05);多层螺旋CT特异度、阴性预测值、诊断符合率、灵敏度高于常规CT诊断,数据差异大(除特异度外,P<0.05)。结论多层螺旋CT具有较高的阴性预测值、符合率、灵敏度,可以提升疾病检出率,对后续针对性治疗指导价值高,可加以推广。Objective To explore the application value of multi-slice spiral CT in the differential diagnosis of pulmonary interstitial lesions.Methods From July 2021 to July 2023,60 patients with suspected pulmonary interstitial lesions were selected from the Radiology department of Zhouzhuang Hospital of Jiangyin City.The diagnostic value of conventional CT and multi-slice spiral CT was analyzed with pathological diagnosis results as the gold standard.The detection rate,diagnostic coincidence rate,sensitivity,specificity and negative predictive value of the above diagnoses of pulmonary interstitial lesions and leek interstitial lesions were analyzed.Results Among the 60 patients with suspected pulmonary interstitial lesions,50 cases were detected by pathological diagnosis,49 cases were detected by multi-slice spiral CT and 41 cases were detected by conventional CT.The detection rate of multi-slice spiral CT was higher than that of conventional CT,and the difference analysis was significant(P<0.05).There was little difference between multi-slice spiral CT and pathological diagnosis(P>0.05).The specificity negative predictive value,diagnostic coincidence rate and sensitivity of multi-slice spiral CT were higher than those of conventional CT diagnosis,and the data were significantly different(P<0.05,except for the specificity).Conclusion Multi-slice spiral CT has high negative predictive value,coincidence rate and sensitivity,which can improve the disease detection rate,and has high guiding value for subsequent targeted treatment,so it can be popularized.
