摘 要:目的观察糖尿病肾病患者空腹血糖、糖化血红蛋白检测及其24h尿总蛋白的相关性研究。方法选取2021年7月—2023年10月本院接收的糖尿病肾病患者60例作为实验组,其中30例合并肾病作为糖尿病肾病组,30例作为非肾病组,另选取健康人60例作为对照组,三组均进行糖化血红蛋白、空腹血糖、24h尿总蛋白的检测,对比组间及组内检测结果情况。结果实验组糖化血红蛋白、空腹血糖、24h尿总蛋白水平均高于对照组(t=15.150,88.610,13.350;P<0.05);肾病组糖化血红蛋白、空腹血糖、24h尿总蛋白水平均高于非肾病组(t=10.400,7.495,21.930;P<0.05);患者体内糖化血红蛋白水平≥9.0%时糖尿病肾病的检出率高于<9.0%(χ^(2)=9.320,P<0.05);患者体内空腹血糖水平≥11.1 mmol·L^(-1)时糖尿病肾病的检出率高于<11.1 mmol·L^(-1)(χ^(2)=9.320,P<0.05)。结论糖化血红蛋白、空腹血糖、24h尿总蛋白的含量与糖尿病肾病的严重程度呈正相关关系,其诊断效果显著,安全性好,应多加推广应用。Objective To observe correlation between fasting blood glucose(FBG),glycosylated hemoglobin(HbA1C)and 24-hour urinary total protein(TWP)of patients with diabetes nephropathy.Methods The paper chose 60 patients with diabetes nephropathy from July 2021 to October 2023 as experimental group,including 30 cases in diabetic nephropathy group,and 30 cases in non nephrotic group,and chose 60 healthy cases as control group.Glycosylated hemoglobin,fasting blood glucose,and 24-hour total urinary protein was detected for three groups,related results were compared between groups.Results Levels of glycated hemoglobin,fasting blood glucose,and 24-hour total protein in experimental group were higher than control group(t=15.150,88.610,13.350;P<0.05).Levels of glycated hemoglobin,fasting blood glucose,and 24-hour total protein in renal disease group were higher than non renal disease group(t=10.400,7.495,21.930;P<0.05).Detection rate of diabetes nephropathy was≥9.0%when glycosylated hemoglobin level of diabetic nephropathy patients was<9.0%(χ^(2)=9.320,P<0.05).Detection rate of diabetes nephropathy was≥11.1 mmol·L^(-1) when fasting blood glucose level of diabetic nephropathy patients was<11.1 mmol·L^(-1)(χ^(2)=9.320,P<0.05).Conclusion Contents of glycosylated hemoglobin,fasting blood glucose,and 24-hour urine total protein are positively correlated with severity of diabetes nephropathy,with significant diagnostic effect and good safety,which should be promoted and applied.
