微血管减压术治疗面肌痉挛的疗效及并发症分析

摘　　要：目的本文主要对面肌痉挛行微血管减压术的手术疗效以及相关并发症进行简要探讨。方法选取2010年1月—2022年6月入住南通市中医院神经外科行微血管减压术的治疗面肌痉挛患者44例为研究对象,进行为期一年的随访,观察所有面肌痉挛患者行微血管减压术的治疗效果及出现并发症的情况,进行统计和相关危险因素分析。结果观察所有患者行面肌痉挛微血管减压术治疗效果情况,44例行手术治疗的面肌痉挛患者中,有效人数为42例,总有效率为95.4%,无效人数为1例,无效率为2.3%;观察所有患者术后出现并发症情况,患者围手术期发生谵妄、肺部感染、口周疱疹、脑脊液漏、面瘫、耳鸣、听力损伤等并发症,有3例患者遗留轻度面瘫,发生率6.8%(3/44);2例患者有不同程度听力下降,发生率4.5%(2/44)。结论微血管减压术是目前治疗面肌痉挛的主要手术方式,具有安全性高、治疗有效率高等特点,但也会发生不同的并发症,本文对并发症情况及原因进行简要探讨,以提升手术技巧,减轻手术并发症的发生。Objective This article mainly discusses the surgical effect and related complications of microvascular decompression for hemifacial spasm.Methods A retrospective study was conducted on 44 patients with hemifacial spasm who were admitted to the Department of Neurosurgery of Nantong Hospital of Traditional Chinese Medicine from January 2010 to June 2022.A one-year follow-up was conducted to observe the therapeutic effect and complications of all patients with hemifacial spasm undergoing microvascular decompression,and analyze the statistics and related risk factors.Results The effect of microvascular decompression for hemifacial spasm was observed in all the patients.Among the 44 patients with hemifacial spasm treated by surgery,42 cases were effective,the total effective rate was 95.4%,one case was ineffective,and the inefficiency was 2.3%.Postoperative complications were observed in all patients,delirium,pulmonary infection,perioral herpes,cerebrospinal fluid leakage,facial paralysis,tinnitus,hearing damage and other complications occurred in the perioperative period.There were 3 patients with mild facial paralysis,the incidence rate was 6.8%(3/44);2 patients with varying degrees of hearing loss,the incidence rate was 4.5%(2/44).Conclusion Microvascular decompression is the main surgical method for the treatment of hemifacial spasm at present.It has the characteristics of high safety and high treatment efficiency,but different complications may also occur.This article briefly discusses the complications and their causes,so as to improve surgical skills and reduce the occurrence of surgical complications.
